We are seeking to appoint a highly motivated individual who would relish the opportunity to work in the adult Learning Disability Service within LPFT as a Learning Disability Physical Health Liaison Practitioner.

LPFT's adult learning disability service, provides specialist health support to people with learning disabilities who require assessment and/or treatment for their physical and mental health, including support with behaviours of concern. The adult learning disability service operates from three multi-disciplinary hubs, across Lincolnshire, which delivers a range of specialist provision as well as a county-wide community home assessment and treatment team.

Main duties of the job

This is a community and hospital-based role providing physical healthcare support and liaison to professionals both within the Learning Disability service and partner agencies including GP practices and services in United Lincolnshire Hospital Trust (ULHT).

You will be responsible for ensuring a high standard of physical healthcare support is provided for service users with a Learning Disability whose complex physical healthcare need cannot be met without the support of specialist Learning Disability services.

The role involves managing a small complex caseload of patients who have a learning disability. You will work closely with the ULHT safeguarding lead nurse coordinating patient care providing support to the Acute liaison nurses and with primary care colleagues to support Community liaison nurses.

As part of this role there is a requirement to support the validation of GP Quality Outcome Framework Learning Disability Registers annually. Part of the role will involve engaging with practices whose registers are not part of the Validation process to understand if additional support is required.

You will deputise in the absence of the physical health liaison lead and provide management and clinical supervision to staff within the service and provide support and training in respect of community and acute physical healthcare to the Learning Disability Service.

To provide support and advice for individuals with a LD who also have a physical healthcare condition who are admitted too ULH hospitals.

To needs led interventions based on assessed needs which are measured against agreed outcomes,

To provide support to ULHT when cover the ALN nurses are on annual leave.

To support individuals with learning disabilities to have their health needs met across all health care settings, including planned admission/appointments to hospital.

To work with patients who do not attend Annual health check appointments and /or NHS screening programs to establish the reason for the non-attendance and provide advice support to the individual with any reasonable adjustments if appropriate.

To work flexibly to support the service.

To provide expert advice and support to clinicians within LPFT and beyond in regard to meeting the physical health care needs of people with a learning disability

To work in collaboration with UHLT and primary care to ensure that the individuals with a learning disability are supported utilising the Mental Capacity Act to make decisions relating to their care and treatment.

Participate and/or coordinate best interest meetings.

The core hours of the role are 9-5pm Monday to Friday but there may be the need on occasions to work flexibly to support the service.

Provide management and clinical supervision as required to peers and junior staff in line with the Trust policies and protocols for supervision, appraisal, and mandatory training.

To ensure a high standard or clinical practice and that clinical skills are updated as required.

To manage a complex caseload ensuring that records are kept in line with Trust and service expectations.

Person Specification

Qualifications

Essential

  • 1st Level Registration (NMC). (RNLD, RMN, RGN) or degree equivalent Allied Health Professional registration.
  • Mentorship Module
  • Evidence of specialised continued professional training (degree level) in related clinical practice.
  • Evidence of continuing professional development demonstrating a higher level of clinical practice within this specialist field.


Desirable

  • Leadership training
  • Teaching qualification


Experience

Essential

  • Have experience of working with adults with a learning disability in healthcare settings
  • Experience of working with people with Complex Physical Health needs within community and /or acute health care settings.
  • Experience of working with people with complex needs, behaviours of concern
  • Experience of managing dynamic risk within a health care setting.
  • Experience of delivering management and clinical supervision.
  • Experience of identifying key factors that need to be considered when decisions are made using the Mental Capacity Act
  • Involvement in the completion of clinical assessments and develop plans of care that reflect assessed needs.


Desirable

  • Experience of conflict management within a professional setting.
  • Involvement and completion of decision specific capacity assessments and providing information to support patient understanding.
  • Experience of delivering training sessions to a multi professional audience

  • Ability to travel independently between sites in the Lincolnshire area.
