Community Psychiatric Nurse-Band 6: (Nurse Practitioner):
The Wandsworth Home Treatment Team is an intensive community mental health service providing care for the acutely unwell within their own homes. We are HTAS's Accredited .We are currently looking for a full time Nurse Practitioner Band 6, who is motivated and committed to a be part of a high-quality service working alongside, professionals, carers and service users to be committed to a high quality standard for patient care.
We pride ourselves in building good rapport with patients and have comparatively low rates of serious incidents.
Main duties of the job
Wandsworth Home Treatment offers an alternate to acute psychiatric admission and the team acts as 'Gatekeepers' to the acute wards. The team works closely with the Trusts in-patient's wards. The Wandsworth Home Treatment team helps to facilitate early discharge and provides discharge support to clients in Wandsworth. Wandsworth Home Treatment Team is also able to signpost clients to alternative services.The successful candidate must hold a Diploma or Degree as a RMN; have post registration experience including experience within mental health settings. The candidate will be part of the multi-disciplinary team, working mostly independently. You must be able to have good communication, be organised and possess good time management skills. A car driver/owner is essential

The Wandsworth Home Treatment Team functions 365 days a year providing care for both clients and their carers. The Service is available to adults both Male and Female from 18 years old and upwards addressing an array of mental health difficulties and crisis Intervention. Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting and we also operate in nearly 100 other locations throughout the UK. This is easily accessible via the Northern Line (Tooting Bec Station) and via private transport. The Trust provides subsidised parking fares for all our substantive staff. Our main site is located immediately next to Tooting High Street where staff can easily access a range of shops, cafs and restaurants.

The Deputy Nursing Team Leader/Clinical Charge Nurse/CommunityPsychiatric Nurse/Practice Development Nurse role is suitable forRegistered Nurses who have successfully completed preceptorship and have registered as an approved NMC mentor.
The post holder will deliver, or advise on the delivery of, high quality safenursing care, ensuring consistently high standards of practice and clinicalexcellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.
As a Deputy Nursing Team Leader/Clinical Charge Nurse/CommunityPsychiatric Nurse/Practice Development Nurse you will promote anddevelop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care. You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment. Your professional and personal development will include further postgraduate study and you will be encouraged and supported by your line manager to undertake Masters programmes.
Person Specification Education and Qualification Essential
- RMN (current NMC registration)
- Evidence of continuing professional and personal development
- Full and current drivers licence and car owner (or willing to take part in car lease scheme run by the Trust)
- Mentorship ,ENB 998 or equivalent
- Diploma in cognitive behavioural therapy/psychodynamic counselling
Experience and Knowledge Essential
- Demonstrable experience of working with people who experience serious ongoing mental health problems in a community or outpatient setting post qualification
- Experience of working in a multidisciplinary team
- Experience of assessing and managing crisis with those with a severe mental illness
- Experience of working as a clinical supervisor
- Experience of managing and supervising staff
- Experience of contributing to policy and procedure development and implementation
