We welcome applications from nurses that have at least 18 months of acute hospital experience at a band 5 level with a substantive background in caring for patients with Endocrinology conditions including Diabetes Mellitus, Addison's disease, hyperthyroidism/hypothyroidism and can demonstrate knowledge of caring for patients with diabetic foot symptoms. They must have the ability to care for acutely unwell patients and patients who are end of life. They should be well organized have good administrative and interpersonal skills because many of our patients have very complex needs and require careful discharge planning.
Candidates will need to have excellent communication skills and be able coordinate care of patients across the multidisciplinary team to ensure high standards of evidenced based care for every patient. They should have experience in organising complex discharges and working with community partners including the palliative care team.
The post holder will need to be motivated in supporting and developing the junior team on a shift to shift basis as well as supporting the Ward manager to ensure patient safety and maintain standards on the ward. They must be a positive confident role model, undertaking hands on clinical work as well as supervising and monitoring practice. They must possess a natural desire to drive improvement initiatives, be dedicated and flexible to cover the needs of the service.
Main duties of the job
To anticipate the care needs of patients and actively manage the patient's journey through the unit, including advanced life support and assessment.
To support, and work with the Ward Manager to provide effective unit management. Specifically this will include: staff management, stocks and patients care delivery
To pro-actively participate in the planning, assessing, implementation and evaluation of patient care as part of the multi-disciplinary team from admission to discharge.
To deliver care in accordance with the NMC Code of Practice/Scope of Professional Practice and the Trust's policies and protocols.
To act as an advocate for the patient and family at all times.
To ensure the safety of patients in accordance with local and national safeguarding procedures.
To participate in auditing effectiveness and incorporate finding into practice.
Be aware of own limitations and able to escalate issues appropriately

It is supported by a large multidisciplinary team including Diabetic specialist nurses, therapist, tissue viability and the palliative care team. We are looking for a dynamic and forward thinking nurse with an interest in developing their leadership skills to support and manage a team of nurses to provide high quality patient care.
