We are thrilled to present an exceptional fixed-term opportunity as a Lead Wellbeing Practitioner within the Southeast London Integrated Care System. We are actively seeking an experienced and autonomous individual to join our LD and Autism (LDA) STOMP team, providing holistic care and positive support for STOMP-eligible service users. Your role will be crucial in reducing unnecessary psychotropic drugs in primary care and community settings and making a positive impact on the quality of life of individuals with learning disabilities and autism.
As the ideal candidate, you should be self-motivated and possess extensive experience working with LDA service users in the community or primary care settings. Your expertise in service user case management and supporting NHS England's STOMP and STAMP priorities will be essential. A secondment opportunity is available for current SLaM and Oxleas employees provided you have your line manager's approval. Joining our team provides an exciting opportunity to actively contribute to the reduction of psychotropic drug use among individuals with learning disabilities and autism, enhancing their overall well-being.
