This post is for a qualified ACP with a passion for primary care within the community. You will bring your clinical expertise to a wider multidisciplinary team.

Clinical support and supervision are available for all staff.

Main duties of the job

To be a clinical expert and senior clinician in the field of Primary Care.

To work with the PCN Lead ACP and wider team to continually improve the quality of care provided including the continuation of service improvement to demonstrate better outcomes for our patients.

Your Health Partnership (YHP) is a cutting-edge Primary Care Network working as part of Sandwell and West Birmingham Hospitals Trust.

The successful applicant will need to share the same values of YHP and work as part of a large multi-disciplinary team including GPs, Nurses, Clinical Pharmacists, Allied Health Professionals and Administrators.

    Qualifications

    Essential

    • MSc level qualification in Advanced Clinical Practice
    • completion of recent leadership / management or evidence of equivalant competence evidence of contemporaneous CPD
    • Independent prescriber qualification or working towards


    Desirable

    • leadership / management course
    • Evidence of chronic disease management


    Experience

    Essential

    • Significant recent experience in a senior nursing or allied health professional role within a primary care or emergency setting
    • Experience of leading clinical teams and professional agenda
    • Experience of leadership role and change management
    • Proven track record in improving patient experience and gaining patient/public view
    • Recent experience of direct involvement in delivering the quality agenda
    • experience of telephone triage
    • experience of effective interagency working


    Desirable

    • experience in clinical systems e.g system one
    • experience of multi skilled approach top chronic disease management
    • evidence of innovative practice
    • experience of working within an ACP post within primary care or community care
    • experience of working as or towards autonomous practice

    • Advanced interpersonal and communication skills with an ability to work effectively with diverse populations
    • Demonstrate self belief, self awareness, and ability to self manage
    • Ability to deal with complaints sensitively, avoiding escalation where possible
    • critically analyse complex information and evaluate a range of options to identify service improvement practice
    • Demonstrate ability to make recommendations to improve practice to improve efficiency whilst also improving patient experience
