Are you a paediatric nurse with experience or interest in managing asthma, eczema, or constipation? If so, we have an exciting opportunity for you to join the Patch Children's Community Nursing (CCN) team as a Band 6 nurse.
The Patch CCN team was developed as part of the Children and Young People's Health Partnership (CYPHP), an innovative quality improvement programme aimed at enhancing the health and well-being of children in Lambeth and Southwark. Our team provides community-based care to children and young people with three ongoing conditions: asthma, eczema, and constipation.
We utilise a population-based approach to healthcare and employ progressive technology to deliver services through community clinics located across multiple sites in Lambeth and Southwark.
We are currently recruiting for a full-time position.

Main duties of the job
We are seeking a dynamic, forward-thinking, and resilient children's community nurse to join our team. Ideally, you will have experience working as an autonomous practitioner within acute, community or primary care settings. This role requires excellent organisational, time management and communication skills.
This is an exciting opportunity to deliver a population-based approach to healthcare while working within a well-supported and friendly team. You will have the chance to contribute to ongoing service development and the integration of care into the wider children's community nursing service.
As an autonomous practitioner, you will provide high-quality specialist nursing care, education and training to children and young people in various settings, including homes, schools, nurseries and the community.
Your responsibilities will include:
You will work collaboratively across the children's community nursing service, ensuring the safety and well-being of service users. At all times, you will promote dignity and human rights by adopting person-centered care principles.
