An opportunity has arisen for a Respiratory Nurse/Practitioner to join the Respiratory team to support the in-house Respiratory nursing service and the Virtual Hospital.
The post-holder will be responsible for the management of a defined caseload patients/service-users within the respiratory service and will provide specialist nursing support, advice, education and/or care to that specific group.
The post holder will work closely with the virtual hospital to review and refer to the virtual hospital team for consideration of acceptance into the service.
They will carry continuing responsibility for the assessment of needs, the development, implementation, and evaluation of programmes of care and the setting of standards for respiratory patients.
The successful candidate must be able to demonstrate previous substantive experience of working at a band 6 level and display the motivation and skills to support the current existing team in collaboration with senior nursing and organisational staff.
As part of the role, you will also be seeing inpatients face to face on the wards assessing these patients in relation to Interstitial Lung Diseases (Asthma and COPD) as well as other respiratory requirements for example oxygen and nebuliser assessments.
The role is for a full time post (37.5hrs a week)
The interviews will be undertaken week commencing the 30th September 2024.
Main duties of the job
Made more difficult by national expectations around NACAP audit and ensuring all patients admitted are discussed at the ABC MDT, have care bundles completed, education provided, inhaler technique assessed etc.
Furthermore, we will also be identifying 'fresh' patients in the emergency department who would be suitable for immediate on-boarding into the virtual hospital to avoid admission. Again, we need specialist nurse time to support these patients in their rapid journey from our emergency department to home with Virtual Hospital monitoring.
West Herts has revitalised how it manages airways disease, with daily airways disease MDT's held at lunchtime. Every acute admission needs to be reviewed by the specialist nurse team and presented at the MDT.
Given the volume of acute admissions, we are seeking additional as such, the post-holder will be responsible for the management of a defined caseload patients/service-users within the respiratory service (as described above), provide specialist nursing support, advice, education and/or care to that specific group. This will include for example the monitoring of blood results, and oxygen assessment and prescribing of oxygen.
