We have an Overnight Community Nurse part time position available within the DN Night Service, based in Taunton.This position is nights only working 10 hour shifts. We are a seven day a week service so the post holder will have to work some weekends and bank holidays. The post is 15 hours a week but you will work three 10hr shifts over a fortnight.You'll need to be a car driver with your own vehicle to undertake visits although there is a pool car available routinely. Main duties of the job

As an experienced Community Nurse, you will provide unplanned, urgent nursing care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines as required.

You will work closely with the Overnight DN Hub, with a focus on preventing avoidable acute admission. You will be competent in a broad range of nursing skills, including, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management and medication administration.

You will need to make autonomous clinical decisions and create personalised care plans as required.
