An exciting opportunity has arisen for an experienced clinician to join a dynamic team of experienced clinicians, to work across the North West London region as a Clinical Lead.

Our clinical leads draw on their expertise and the expertise of our partners, to provide independent challenge, oversight and support to CCGs and clinical teams which helps move patients on. The team have a strategic role in improving outcomes, informing new developments, protocols and cultures across NWL, informed by the leadership of patients and clinical staff, the challenges actually faced on the ground and your own expertise. You will be empowered to challenge our thinking and create change that improves our patients lives.

Main duties of the job

MHPP is a small, agile team dedicated to improving patient care. We take pride in the work we do and the change we make. In 2022, we will be doing more to ensure we are patient led and accountable - this is an exciting time of change and an opportunity to join a team determined to create a different way of doing things in the NHS.

You will be involved in multiple cases, meaning attention to detail and organisation is critical - but will also need to be able to take a step back and think about our broader strategy, and keep an eye on cost. We are privileged to work with brilliant partners across NWL, and nurturing relationships will be a central part of your role, as will be the need to influence and challenge at times. Above all, you will be empathetic, patient focused and not afraid to do what is right rather than what is easy. You will be a registered healthcare professional with:
  • Substantial experience as a mental health practitioner, including experience at band 6
  • Experience of carrying out specialist comprehensive assessments and reviews
  • Ability to support, manage and advise colleagues on challenging situations
  • Knowledge of the legislation relating to section 117 of the Mental Health Act
  • Ability to plan and monitor care pathways against expected outcomes
  • Strong negotiation and conflict management skills


About us

You will be part of the North West London Mental Health Partnership Programme (NWL MHPP), an exciting new approach to delivering mental health care for our patients. Our team is made up of clinicians and managers who support the redesign of mental health inpatientplacements, including rehabilitation services and longer term bedded support. Our aim is to make sure patients are supported as close to home as possible, in the least restrictive settings for as short a time as clinically appropriate - and make sure the system is less dependent on these placements in the first place.

A key part of the role is undertaking clinical reviews of in and out of borough placements commissioned by the North West London commissioners. This will include finding suitable, local placements where required, and ensuring that existing placements offer patients the best outcomes. In addition, you will contribute to the wider system work across NWL of identifying and addressing gaps across the whole pathway.

Job description Job responsibilities

Job Purpose:

The post holder will be responsible for:
  • carrying out health and social care needs assessments
  • planning regular reviews
  • ensuring funded placements are achieving best outcomes and value for money
  • arranging transfer from out of area services to local community placements and independent living where appropriate
  • supporting clinical teams in CNWL and West London NHS Trust (WLT) and other placements programmes in CCG and local authorities
  • linking with other projects or programmes which are working to reduce reliance on out of area placements, giving support and guidance
  • advising the Clinical Manager on community resources needed to reduce the use of placements
  • supporting clinical teams to reduce dependency on placements
  • giving professional advice and support to colleagues from different professional backgrounds

Main Responsibilities:

Clinical
  • Complete comprehensive assessments and reviews for mental health service users who are placed out of area
  • Provide expert advice, support and guidance to care coordinators by monitoring move on plans and outcomes
  • Act as expert for staff teams across CNWL and WLT in relation to placements and section 117 aftercare.
  • Work collaboratively with clinical teams to establish review schedules
  • Work with service users, carers and providers to ensure care plans meet individual needs, with clearly defined outcomes and timescales
  • Involve service users and carers in making decisions about their care needs and inform them of assessment outcomes and panel decisions
  • Facilitate the most appropriate and cost-effective care pathway for step-down and move on
  • Identify appropriate local placements to meet the needs of service users returning from out of area placements
  • Support clinical teams to reduce reliance on out of area placements, by supporting initiatives to avoid the need for placements
  • Proactively negotiate on care package costs, using appropriate tools
  • Establish close links with CCG and local authority commissioners and practitioners to evaluate the efficacy of placements and care packages
  • Promote high quality service provision, social inclusion, personalisation and positive outcomes for service users
  • Identify areas of high quality and best practice in the services areas that can benefit the whole health and social care economy
  • Organise work plans and provide assurance that efficiency targets will be met
  • Maintain detailed knowledge of relevant policy and legislation, particularly the Mental Health Act, Mental Capacity Act and the Care Act.
