ULHT's Frailty Service have an exciting opportunity for a Frailty Advanced Care Practitioner (ACP) to become part of our team at Pilgrim Hospital.
Main duties of the job
Successful candidates must be a qualified ACP, be flexible, innovative, highly motivated and above all be committed to improving the care and assessment of frail older people in Lincolnshire.
Location: Pilgrim Hospital. The post holder will be identified a base site, but will be expected to work across all sites within ULHT to meet the needs of the service. There may also be a requirement to work as needed across partner organisations as part of the vision for an Integrated Frailty Workforce for Lincolnshire.

- Current Professional registration within area of practice.
- Post registration qualification or training relevant to speciality.
- MSc in Advanced Clinical Practice
- Registered Independent Non-Medical Prescriber
- Evidence of teaching, supervision and assessment skills

- Older Adults Fellowship (HEE)
- Mentor/assessor qualification
