An exciting opportunity has arisen for a Band 7 Learning Disabilities - Health Facilitation Nurse Lead, to join the Newham Health Team for Adults with Learning Disabilities (NTPLD).
Main duties of the job
This post is suitable for someone who is passionate about primary health care, specifically promoting annual health checks, cancer screening, raising the profile of adult vaccination programmes and improving access to mainstream health services for patients with learning disabilities.
You will need to be highly motivated, imaginative and committed to improving health outcomes and challenging barriers to healthcare for this population.
The post is hosted by the East London NHS Foundation Trust, but works in close partnership with the CCG and local authority. The post is based within the NTPLD offices, but travel is required to other parts of the borough. You will be required to work autonomously, as well as part of the wider team and with a variety of agencies outside of the Trust. The post holder will be directly responsible for the supervision of the Clinical Support Worker.
The Trust actively encourages professional development through the appraisal process and regular supervision, and you will be offered support to develop your nursing skills within an established team.
