The Luton and Dunstable University Hospital are offering a unique and exciting permanent opportunity to experience both community and hospital working. The Hospital at Home Service work in partnership with community and in house services to deliver an integrated care model. The teams are nurse-led and aim to support patients, with exceptional care and treatment in their own homes when it is clinically safe to do so.

We currently have an exciting new opportunity for a Band 6 in The Pilot Team. The successful applicant will have the opportunity to be part of a nurse-led team in an ever developing service.

The Acute Team

This team supports the patient to remain at home by providing additional support in their own house. The care offered is intensive medical treatment for the acutely unwell. They also help facilitate their patients to leave hospital sooner through the provision of these services. Joining this team will give you the unique experience of community and hospital nursing, working across boundaries and supporting an integrated workforce.

We require flexibility of working patterns across the 24 hour day including evening, day shifts 7 days a week. the service currently start at 7:00 Am and finish at 8:30 Pm

Main duties of the job

The post holder is identified as a Hospital at Home Team Nurse, working in conjunction with the current H@H teams , providing skilled nursing services for patients who are suitable for discharge and to continue their treatment at home. The Hospital at Home service, offers a seamless patient journey across Primary and Secondary Care. The post holder will provide education and training for patients/ carers and members of the multidisciplinary team across boundaries share nursing knowledge and skills to support and advice as needed. The post holder will develop effective working relationships within Primary and Secondary Carecolleagues, GP's and other Primary Health Care Teams to facilitate integrated care and enable early supported discharge, prevent inappropriate re-admission to hospital. Support and participate in the development of integrated pathways and care packages which enablepatients to be safely cared for outside of the acute hospital setting. Provide clinical leadership and clinical management of the service 24 hours 7 days a week and continuously monitor and maintain clinical standards and quality care. Monitor effectiveness of protocols and patient triage requirements and escalate level of care for acutely ill patients wherenecessary.

Person Specification

Qualifications

Essential

  • Qualified Nurse
  • Evidence of continuing professional development
  • MSPP or evidence of mentorship updates and a willingness to mentor or co-mentor students


Desirable

  • Knowledge in coagulation
  • History taking and Assessing course


Skills

Essential

  • Competent in holistic patient assessment.
  • Teaching and presentation skills
  • IV administration , venepuncture and annulation, CVA access
  • Ability to write reports and use IT


Desirable

  • Experience of clinical supervision


Experience

Essential

  • Experience working as Band 6 within an acute specialism or Band 5 with long nursing experience and evidence working in acute setting
  • Experience of supervising junior staff
  • Some managerial experience


Desirable

  • Experience in managing and delivering sustained change
  • Community experience
