To work as part of a multi-disciplinary team to safeguard, promote and support the health and wellbeing of children in care following statutory guidance, legislation and Wye Valley Trust polices and procedures.

Undertake statutory health assessments and develop individualised high quality health action plans for children and young people who are in care.

Support the health of children and young people in care when there are new or changing concerns about the health of a child or young person in care.

Main duties of the job

Organise, undertake and co-ordinate, holistic health assessments that consider the physical, emotional and social of children and young people in care.

Prioritise new and unmet health needs for children in care making appropriate referrals and liaise with identified professionals to ensure the health care for all children in care is timely and any disruption to ongoing health care is minimalised.

Support and empower children and young people to age appropriately understand their health and any medical diagnosis and encourage their contribution in the development of their individualised health plan.

Develop individualised clear health action plans that are child centred and evidenced based.

Provide age appropriate health promotion, advice and education to children and young people in care.

Communicate with children/young person's where there may be a variety of barriers tounderstanding and communication.

Actively promote good health and wellbeing in areas such as sexual health, smoking, drugs and alcohol and with the young person's consent signpost to specialist health services.

Education & Qualifications

Essential

  • Registered General Nurse (1st level)
  • Specialist Practitioner Public Health Nursing


Desirable

  • Registered Sick Children's Nurse
  • Nurse for Children with Disabilities
  • Registered Mental Heath Nurse


Skills, Knowledge & Abilities

Essential

  • Exceptional interpersonal and communication skills in order to provide and receive highly complex and sensitive information
  • Able to assess, plan, develop, implement and evaluate health care plans
  • Plan and prioritise own workload
  • Flexible/and adaptable approach to work
  • Computer Literate
  • Good organisational skills
  • To be proactive and have a forward thinking approach


Experience

Essential

  • Experience of working with children and young people
