An exciting opportunity has arisen to join the Cardiology Advanced Clinical Practitioner (ACP) team on the Clinical Decisions Unit (CDU) at Glenfield Hospital. CDU admit patients with a wide variety of Cardiac conditions. The ACP team work with the Cardiology medical workforce to provide specialist care for these patients.

We are looking for an ACP preferable with Cardiology experience, or someone who relevant experience and is willing to develop their skills in this area. This is a small, supportive team which offers great opportunities for learning and development.

Main duties of the job

The post holder will be an integral member of the Cardiology workforce on the Clinical Decisions Unit at Glenfield Hospital, working collaboratively with medical, nursing, midwifery and allied healthcare professionals undertaking advanced roles and responsibilities traditionally the remit of medical staff under the direction of the Consultant in charge of the patients care.

The Advanced Clinical Practitioner (ACP) will work within agreed protocols and guidelines exercising independent judgement in history taking, examination, assessment, diagnosis, and treatment of a specific and undetermined caseload of critically or acutely or chronically ill or injured patients who may have highly complex acute presentations to the more minor common illnesses and injuries.

The post will have a primary location but may extend and influence across UHL, including the Emergency Department and adult inpatient wards.

The ACP will act as a professional resource for the multi professional teams, patients and carers. They will ensure that evidence based practice is inherent in all aspects of care and treatment and ensure this knowledge is disseminated throughout the clinical area, across the Trust and externally as appropriate.

Job responsibilities

Service Delivery & development: to comprehensively clinically assess patients who attend the Glenfield Cardiac Rhythm Service in line with evidence based practice, legislation, policies and procedures. Provide treatment plans for patients that are consistent with the outcome of the assessment. Initiate diagnostic tests, interpret and act upon the results. Practice autonomously, in line with departmental policy and competence.

Governance: to undertake audit, education, participate in research. To maintain comprehensive, and accurate assessment and treatment records. Recognize any limitations in knowledge and competence, referring to colleagues, both senior ACPs and Consultants.

Patient Service: Act as a specialist resource for the Centre. Improve the quality of care and patient experience by identifying and meeting individual needs. Act as a patient advocate.

Person Specification

Education & Qualifications

Essential

  • Registration on relevant part of the register with the NMC or HCPC with no restrictions on practice
  • Master's degree level education meeting the requirements of the national framework for Advanced Level Practice
  • Have completed a Practice Assessor preparation day and have recent experience in supporting learners in the workplace and supervising their progress and assessing their practice
  • Research module as part of Master's degree
  • Independent non-medical prescribing (V300) if permitted within professional group


Desirable

  • Clinical based Master's degree
  • Study/knowledge relevant to speciality
  • Post graduate management / leadership qualification or willingness to undertake
  • Study/knowledge relevant to speciality
  • Counselling qualification
  • Research qualification
  • Have successfully completed a previous NMC Approved Teaching and Assessing or equivalent


Advanced clinical skills

Essential

  • Evidence of developing advanced skills in history taking and physical examination skills within speciality area
  • Highly developed communication skills
  • Ability to assess, diagnose and action under pressurised environments
  • Demonstrates expertise in complex decision making
  • Autonomous management of own patient load (ward based or clinic environment)
  • Autonomously assess, diagnose and develop management plans for urgently referred patients

Experience

Essential

  • Highly developed specialist experience across a wide range of emergency care procedures that is underpinned by theory and continued knowledge and skills through CPD
  • Experience of prescribing practice in the last 12 months
  • Evidence of involvement in policy and protocol development and implementation and ability to influence service changes
  • Experience managing own caseload of patients and autonomous practice
  • Experience of teaching, assessing and mentoring of learners
  • Experience in audit, research and development
  • Proven line manager experience able to manage a team of healthcare professionals


Desirable

  • Lead in Audit or Research projects
  • Evidence of developing nurse or therapy led services and clinics in collaboration or independently of medical consultants
