We are looking for a Consultant Advanced Clinical Practitioner to join ourEast Kent HCP Frailty/Home Treatment Service.
You will will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people within a variety of healthcare settings. S/he will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of the older person.
Main duties of the job
You will lead and monitor improvements to quality of care through clinicalaudit, evidence based practice, clinical supervision, teaching and supportingprofessional colleagues and the provision of skilled professional leadership.
You will lead the implementation of relevant clinical guidelines into practice and contribute tothe further development of the evidence base through research and audit.
You will lead the development of non-medical led models of care acrossEmergency departments, inpatient wards, intermediate care, care homes and community teams in collaboration with multi-professional colleagues.
You will play a crucial role in improving patient flow through timely assessment, referral, responsive intervention, anticipatory care planning and optimal discharge coordination across seven days

QUALIFICATIONS

- o First level Registration with NMC, HPCP or GPHC o Minimum of 5 years post registration experience, Experience at band 8a or above o Is an Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice OR Successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met. o Maintained a portfolio of ongoing evidence or work based learning. o Teaching qualification o Research qualification o Leadership o Non- medical Prescribing
