We have an exciting opportunity in NMUH within the Respiratory team in the development of our newly created Respiratory CNS team. We are looking to recruit our Lead Nurse for Respiratory CNS team that is an experienced, motivated, passione in regards Asthma and COPD .
This is a clinical leadership role in the team providing scope for innovative work with various stakeholders and improve patient outcomes and experience. Within the team, supporting the development of the team of CNS role including with prescribing.

There are opportunities for flexible working as you will be provided with the tools to deliver virtual consultations, but you majority of the work will be supporting our patients in ED, Inpatient and Outpatient setting.
As this is a new Team the Lead Nurse Respiratory will be setting up the service to match the demand and capacity of the patients in NMUH with the support of General Manager/Service Manager, Respiratory Consultants and Associate Director of Nursing.
Further opportunities for training and personal development will also be available for this role.
Main duties of the job
The post holder will take devolved responsibility for the care of patients across inpatient and outpatient settings. They will be responsible for assessing care needs and carrying out all relevant forms of care without direct supervision. This will involve case management, running nurse-led clinics, a telephone hot-line, supporting patient self-management, providing education to other healthcare professionals involved in the care of respiratory patients and strengthening links with community teams. The focus of work will be supporting the care of people with asthma and COPD.
The post holder will develop a strategy in collaboration with the medical team to improve patient experience and quality of care as well as reducing admissions and overall length of stay. Work will include supporting local and national audits, support to ward nursing staff in the care of respiratory in-patients and collaboration with community colleagues to ensure joined up care between the hospital and patient's homes and improved access to pulmonary rehabilitation.
The Lead Nurse will be involved in guideline/audit development and be expected to write and update local nursing policies and evaluate their implementation. The post holder will be expected to demonstrate excellent communication and clinical skills and support teaching for relevant healthcare professionals to improve the care of respiratory patients.

- Manage and co-ordinate the care for patients referred with COPD and Asthma.
- Be responsible and accountable for all elements of professional nursing practice.
- To work autonomously in managing patients eligible for biologics and liaise with medical team and members of the multi-professional team.
- Maintain effective communication with patients, carers, and professionals to ensure seamless service delivery.
- Assist with the assessment of new patient referrals, working alongside consultants to ensure appropriate investigations are performed in a timely manner and an agreed management plan is in place.
- To undertake assessment of the patient which includes taking a history, physical examination, and when applicable, ordering and interpreting diagnostic tests as appropriate.
- Ensure all assessments are documented fully and communicated effectively to consultant, referring member of the medical team or GP and members of the multi-professional team.
- To review previous test results and identify any abnormalities with blood tests / imaging and instigate appropriate action or escalation to consultant.
- To be accountable for coordinating patient care with the consultants and administration team, in order to meet agreed standards/pathways. This will include prescriptions and updating patient and patients referring consultant or GP of the care/treatment plan.

Management
- Co-ordinate a nurse-led respiratory service which promotes a safe patient pathway for those eligible patients. Work collaboratively with Clinical Lead and Service teams to optimise demand and capacity within respiratory.
- To collect, collate and evaluate information relating to patients referred and procedural outcomes. All patient pathways are tracked, and data collated on key events such as timing of receipt of referral, date of consultations, treatment options and clinical outcomes.
- Work collaboratively in developing straight to test triaging/assessment models including research through data collection and analysis of key outcome measures.
- Present information to professional groups, analyse data and provide written reports. Data will be analysed frequently to identify risks or delays in patient care which require immediate intervention by Clinical Lead or service teams.
- To establish and maintain effective communication links with all relevant personnel and departments, external agencies as necessary.
- To liaise with the health advocacy service to book advocates as appropriate to the patients needs.
