Main duties of the job

To provide and implement a plan of care for the client in collaboration with the multi-disciplinary team, referring agent, family/carer and client where appropriate.To review and evaluate care in accordance with C.P.A.To liaise with all agencies, statutory, independent and voluntary that are involved in the care of older people with mental illness.To maintain accurate, up to date records of all clients referred and receiving assessment, care, treatment and support, including the C.P.A informationsystem.To act as supervisor to students placed with the Team.To act as mentor and role model to junior members of the team.To participate in the duty screening rota.To operate within the Policies and Procedures lay down by Pennine Care NHS Trust.

Job responsibilities

Assist in ensuring the delivery of a safe, effective service to an appropriate client group. Assist and participate in the continuing development of a high quality service for older people.In the absence of the Team Leader to assist with the day-to-day decision making in the team as requested.To assist in meeting budgetary targets for all aspects of the service,demonstrating an awareness of the cost implications of utilising the Organisations resources.

To assist the Team Leader in monitoring activity, caseload management, and allocation of work. To report any concerns or under-achievements to the team leader.To demonstrate practice as a clinical expert in the field of mental illness in old age and its management.To provide and/or facilitate clinical supervision in individual or group settings andto all grades of staff. Where necessary undertake live supervision andmentorship. To participate in mandatory individual and group supervision.To be involved in the clinical development of junior staff, including training and development planning updates and research awareness. To notify Team Leader of any training needs identified in yourself or your supervisees.To deliver training to health staff and other agencies as directed by the Team Leader either in conjunction with other staff or individually.To plan, implement and review individualised care plans, ensuring standards are maintained regarding the quality of interventions provided.Provision of reports as required for the purposes of care planning, caseconferences and case reviews.Participate in Multi-Disciplinary Team Meetings, Ward Rounds, CPA reviews, Section 117 reviews and care planning meetings as required.

Person Specification

Education / Qualifications

Essential

  • Registered Mental Health Nurse (RMN)
  • Postgraduate experience and short courses to postgraduate diploma level.


Desirable

  • Trained to degree level
  • Accredited Skills Training
  • Management Training


Experience

Essential

  • Minimum of four years post qualification experience in mental health
  • Experience of acting as a care co-ordinator under the Care Programme Approach
  • Experience in working with older people with severe and / or enduring mental health problems


Desirable

  • Experience of leading a team / 'acting up'
  • Experience of working within CMHT


Knowledge

Essential

  • Ability to provide leadership cover in the absence of the team manager when required
  • Knowledge of mental health legislation and guidance
  • Demonstration of sound communication skills both verbal and written
  • Knowledge and experience of multidisciplinary team working


Desirable

  • Knowledge of managing or participating in change


Skills and Abilities

Essential

  • Ability to manage and monitor staff performance to ensure appropriate standards are maintained
  • Ability to identify and coordinate effective risk management
  • Experience and skills in working in partnership with service users, relatives and carers
  • Skill to provide and receive clinical supervision


Desirable

  • To have developed presentation skills
