Rehabilitation Division have an exciting opportunity for dynamic and innovative individuals who are looking for a challenge, to be part of the newly developed Community Rehabilitation Team. The service aims to provide bespoke, intensive and individualised care to service users living with complex, enduring mental health problems to enable them to live fulfilling lives to their full potential.
The successful candidate will play a central role within the MDT to ensure high quality, recovery focussed care is delivered to our service users in the community.
The role involves care coordination of a small caseload (approx 10) of service users and will lead on the assessment, collaborative care planning and provision of interventions which encourage self management, optimise individuals independence and promotes choice and inclusion as part of their recovery.
The service will initially pilot in Salford, with roll out across Manchester localities thereafter. Full induction and training will be given as part of the role.

Main duties of the job
In the role of care coordinator for a small caseload (10) you will have the opportunity to work with service users who have been identified as requiring community based rehabilitation pathway under the CPA framework.
You will need to be able to establish rapport, trust and the ability to work collaboratively with service users and their carers to support their recovery and thrive in their own homes and local communities.
The role will be varied, and the ability to work creatively and flexibly will be an essential aspect of the job. The post holder should aim to promote the independence and well-being of service users; assist social inclusion and valued community presence and enable the widest possible expression of chosen lifestyle.
You will work with within a full multi-disciplinary team and participate in shared clinical decision making to determine most appropriate management plans for service users. You should be able to work independently as well as part of a closely supported team and have the ability to prioritise tasks. You will have the opportunity play a key role in the development of a new service, and in the organising, supervising and supporting of junior colleagues.
The post holder will be required to work flexibly to cover the service hours of 7 days a week between the hours of 8am-8pm

To demonstrate high-level communication skills with service users who may have complex emotional, physical and psychological conditions.
To establish and maintain robust communication networks with service users, carers, team members, other health care and social care workers and external agencies.
To establish and maintain good working relationships with GPs, and other partner agencies
To be a full member of the multi-disciplinary team (MDT) and to participate in clinical decision making to determine most appropriate management plans for all service users under the team.
To be responsible for coordinating crisis management and creation of crisis plans which identify early warning signs and triggers to ensure timely and proactive response.
To undertake and be responsible for the assessment of mental health and social care assessments, collaborative development of, planning and delivery of recovery focussed goals including strategies to positive risk taking.
To ensure delivery of range of interventions which include the provision of recovery based activity.
To organise and run community based depot and Clozapine clinics; playing a central role in the requisitioning or ordering of necessary equipment and medications, linking in with relevant pharmacy departments.
Follow policy and procedure to ensure safe storage of medication. Administer and monitor medication prescribed to service users providing information and advice.
Complete carers assessments and support families, friends and carers referring to appropriate services as required
To participate in Community Rehabilitation Team duty rota including any emergency assessments

Person Specification Education/Qualifications Essential
- Registered Mental Health Nurse - diploma or degree in Mental Health Nursing
- Trained Mentorship
- Further qualifications/training relevant to SMI e.g. CBT, Trauma Informed, PSI
- Extensive experience of working with adults of working age
- Experience of working with people with complex needs and co-morbidities
- Experience of working in a recovery focussed way
- MDT Working
- Working within rehabilitation services
- Experience of working with adults in acute/crisis/relapse prevention
- Experience of supporting people who misuse alcohol or drugs
- Experience of delivering line management and clinical supervision
- Working knowledge and understanding of CPA
- Knowledge and understanding of recovery model, appropriate therapeutic approaches and engagement strategies
- Knowledge and understanding of Mental Health Act, Mental Capacity Act & Care Act
- In depth understanding current national policy, initiatives, and principles of rehabilitation
- Knowledge and understanding of safeguarding frameworks and procedures

- Have evidence of good time management and organisational skills being able to work under pressure.
