This is an exciting role for a qualified Mental Health Nurse to work within the Bournemouth West Older Persons CMHT based in Dorset.

The CMHT is an integrated service with colleagues from Adult Social Care from BCP Council embedded and based within the office and team this role will be varied and offers opportunities to work with service users suffering from a range of serious mental illnesses, including dual diagnosis from the Older Person population. This is primarily a care-coordination role, where you will be responsible for supporting patients on their journey of recovery as part of a wider multidisciplinary team.

As a key member of the team, you will find the experience challenging, but rewarding and varied within the well-established and supportive team, where we have strong values about working in a patient centred manner, where their recovery and wellbeing is at the heart of everything we do. You can expect close working peer support and regular supervision to enable you to feel safe in your practice whilst further developing your skills.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job

To provide a comprehensive, accessible and responsive mental health service to patients and their families within a defined locality.

To support the team and locality leader in implementation of the Dorset Healthcare Mental Health Strategy.

To continually develop clinical, team and service evaluation and delivery of service.

To contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual's need and service development.

Person Specification

Knowledge, Skills and Training

Essential

  • Registered Mental Nurse
  • Valid registration with NMC or equivalent body
  • Evidence of continuous professional development


Job Specific Experience

Essential

  • Risk assessment and management
  • Able to assess, plan, implement and evaluate programmes of care.
  • Proven communication skills (written and oral)
  • The ability to prioritise and meet deadlines organise work of self and others supervising, teaching and assessing self-motivated with the ability to motivate others effective resource management.
  • Skills in managing complex cases.


Desirable

  • Extensive post graduate experience in working in community mental health.
  • Working experience of using the Care Programme Approach.


IT

Desirable

  • Competent computer skills, experience inputting information on RIO


Additional Requirements

Essential

  • Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
  • Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively.
