- Provide clinical support and direction to community nursing staff within the locality by sharing their expertise.

- Works alongside the Operational Team Lead to ensure staff are competent in their role and new staff and inducted and supported to reach competency required for their role.

- Implementing evidence-based practice to ensure best patient care.

- The function of the Clinical Lead is to provide enhanced clinical support and expertise to the patients within their locality.

- Hold a Specialist Community Nursing Qualification.

- Hold an Independent Prescribing qualification (V300) or being to undertake within 12 months in post.

- Promoting a service which prevents patients from having inappropriate or unscheduled hospital admissions and helps facilitate hospital discharge.

- Plan, organise and deliver regular staff meetings to share information pathways within the service and wider organisation in a robusttimely manner. e.g. Team brief, CAS alerts, and feedback from senior management meeting

- Investigate complaints and incidents, including Serious Incidents requiring investigation,safeguarding and clinical issues.

- Identify the Root Causes, make recommendations for improvement and disseminate learning to staff, adding areas of concern to the risk register.

Main duties of the job

  • Ensure care in their locality is based on sound assessment and care planning processes.
  • I.T. literate to support staff transitioning to mobile working.
  • Positively represent the service both internally and externally. Communicate highly complex information using informal and formal means to ensure people are fully informed of the service.
  • Ensure that appropriate Statutory and mandatory training has been undertaken by all team members.
  • Responsible for the retention and recruitment, induction, personal and professional development of staff.
  • Monitor clinical supervision, induction, preceptorship and development of staff.
  • Exercise leadership and judgment in the management of a Community Nursing Team to ensure good clinical practice and maintain high standards of care, and review staff practice at regular intervals.
  • To provide education opportunities and training for all staff and students including pre- and post- registration nursing students.
  • To develop district nursing practice in a defined area, developing and using relationships with other primary care team members.
  • Ensures that the service is meeting business and performance requirements of the Organisation in-line with service specification, contract and CQUIN.

Person Specification

Education/Qualifications/Training

Essential

  • NMC registered Nurse
  • Specialist Community Practice qualification relevant to field of practice. (DN Degree, diploma or Certificate in District Nursing) Teaching and assessing qualification.
  • Must be willing to undertake Advanced History taking qualification within 6-12 months of appointment.
  • Must be willing to undertake Non-Medical-Prescriber Qualification within 6-12 months of completing advanced history taking.
  • Knowledge relating to practice assessor qualification.
  • Evidence of continuing professional development (CPD)


Desirable

  • Evidence at studying at Masters level.


Previous Experience

Essential

  • Experience in teaching and assessing both pre and post reg. nursing.
  • Act as clinical expert in the field of community nursing
  • Evidence of clinical leadership within a nursing environment.
  • Knowledge relating Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from findings.
  • Able to manage own workload and support others.
  • Able to conduct courageous conversations and support staff with performance related issues.
