An exciting opportunity has arisen to join the tissue viability team at Midlands Partnership Foundation Trust.

This would be an ideal opportunity for the successful candidate to extend on existing skills and develop further skills and knowledge within the tissue viability discipline. The role entails dealing with patients who have pressure ulcers, leg ulcers and other complex wound care needs. The post holder would also be expected to undertake triage duties and wound care projects as part of the role.

Potential post holder needs to have experience in leg ulcer management and existing wound care skills as well as being able to undertake educational programmes to patients and clinicians.

The potential post holder needs to be a committed and confident individual who is conscientious, dynamic and pro-active and has excellent communication and organisational skills.

Main duties of the job

To provide specialist nursing care, intervention and education though a variety of different approaches, to staff and patients with tissue viability needs within Staffordshire, across any care setting. The role will support tissue viability strategic developments as identified by the Tissue Viability Team and the Clinical & Operational Lead providing local specialist wound management and advice, education and training for staff across Staffordshire ensuring an equitable and continuous service between the primary and secondary care settings.

Job responsibilities

Provide Specialist nursing care and management to patients with complex wound care needs in accordance with national guidance and policies
  • Analyse and rigorously review all aspects of the patients programme of care interpreting information and using specialist knowledge and judgement to provide highly specialist advice to recommend new course of action where necessary. The care delivered should reflect national guidelines and standards to support high quality evidence based practice
  • Undertake clinical assessment including formulation and adjustment of care plans within a defined governance framework and reporting structure.
  • Provide a specialist-nursing element to the University Hospitals of North Midlands Trust hospital (UHNM) based clinics as a member of the multidisciplinary team andCombined Healthcare Trust.
  • Work in partnership with Secondary Care to facilitate safe discharge of patients.
  • Facilitate and lead in the delivery of structured and targeted education programme facilitating the development of self-care skills and independence.
  • The post holder will establish and maintain their own workload, maintaining responsibility and accountability for all elements of professional nursing practice.
  • Raising awareness and promoting healthy living in patients with complex wound care needs and the associated complications of the long term condition.
  • To participate in Clinical Harm Incident Review Panel meetings, Safeguarding and vulnerable adult interventions, root cause analysis and any other required wound related processes.
  • To support with the development and implementation of all Education, Policies and Guidelines that is relevant to the role of the Tissue Viability Service.
  • To provide effective support for staff in the provision of holistic tissue viability care and to communicate concerns effectively to senior tissue viability nurses.
  • Facilitating learning amongst health care professionals, locally, nationally and internationally.
  • To undertake appraisals & supervision for junior staff
  • To be professionally and legally accountable for own practice.
  • Act as a role model to professional colleagues in all aspects of clinical practice. Demonstrating professionalism at all times. To adhere to, apply the Nursing & Midwifery Council (NMC) code of conduct, and all other relevant guidance from the NMC, and follow the Trust values.
