37.5 hours per week. An exciting opportunity within Livewell Southwest has become available to join the Tavistock community nursing team as a community nurse team manager on a full time permanent basis.

As an experienced band 6 or existing band 7, we welcome applications from individuals that are motivated and passionate about managing a team of Community nurses and HCAs, Within a rural west Devon locality. West Devon is a beautiful location to work, and has a well established close nursing team. As a Team Manager you will be working closely along our other Health and Social care colleagues, to provide the best outcomes for our community. Main duties of the job

The main duties of the role will include but are not exhaustive of

Daily operational lead over a team of community nurses and HCA's

Ensuring safe allocation of work has been applied across all teams

Line managing a team of band 6's

Being a budget holder and ensuring invoices and expenses are paid

Ensuring staffing is safe and Eroster is updated

Facilitating regular team meetings

Attending regular MDT meetings.

Responding to incidents and any safeguarding risks.

Caseload supervision.

Liaising with other HCPs on a regular basis- GP's, physiotherapists, Occupational therapists, social workers, Care providers.

Ensuring staff wellbeing is being monitored and maintained at all times.

Supporting clinically when needed to the team and student nurses.

Ensuring competencies are maintained and education/training is provided regularly.

7.Dimensions and Context of Role

7.1. The District Nursing Team Manager carries the responsibility for the operational management of their locality based District Nurse Team.

7.2. Has responsibility over a twenty four hour period 365 days for the District Nursing Team and is supported by Band 6 s within the team to achieve this.

7.3. To provide leadership with service development and redesign, to scope and profile local health needs within their locality and to identify available resources required or deficits to action to meet that need.

7.4. Active involvement in the strategic planning of health and community based services and to deputise for and support the Locality/Deputy Manager and Professional Leads.

7.5. Part of the role is to provide clinical leadership and have a clinical role within the team.

9. Key Tasks and Responsibilities of the Post:

Clinical:

9.1. To ensure privacy, dignity and human rights are respected for all Patients and Carers.

9.2. Facilitate the team when dealing with complex/ urgent situations which can be emotionally challenging e.g. continuing NHS healthcare, Safeguarding Adults, child protection issues in partnership with Patients and Carers.

9.3. To support caseload holders in case managing/care coordinating packages of care.

9.4. To be responsible for the quality assurance of continuing NHS healthcare assessments within the Team and across the locality.

9.5. To identify and challenge professional and organisational boundaries, identifying areas for development and apply these to practice to provide continuity and high quality patient centred health care.

9.6. To work with integrated multi-disciplinary teams across health and social care to co-ordinate the development, implementation review and evaluation of care programmes for patients and their carers.

9.7. Ensure that the care provided and services delivered are in line with national and local and organisational targets.

9.8. To undertake health promotion whenever possible to all age groups and also contribute to planned health promotion activities.

9.9. To maintain accurate records, which are confidential, up to date, legible and all care given to be documented. These records may be paper or IT based system and must be maintained as specified in the organisation Policies, NMC guidelines and Government directives.

9.10. The post holder will be expected to oversee the standard of patients records by audit or peer review and have a regular open discussion with team members.

9.11. The post holder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations/ Chief Executives office).

9.12. To monitor and maintain the health, safety and security of self and others.

9.10. Management & Leadership:

10.1. Operational manager for a locality based District Nursing Team.

10.2. Set workload priorities for self and team, achieving a balance across patient care, management responsibilities and the needs to meet staff training requirements within resource limitations.

10.3. To report and record and manage all incidents and near misses relating to health, safety, security, fire, physical violence, aggression and verbal abuse.

10.4. To provide a visible and accessible presence for patients and staff within the organisation.

10.5. Provide professional clinical support to the nursing team and advise on practice, identifying training needs of staff and encourage practitioners to develop:

Skills in critical thinking and reflective practice

Excellence in Practice

Specialist knowledge

Research based practice

10.6. Ensure that structured team meetings are regularly held and documented.

10.7. Identify and lead on clinical audit within own work area. Participate in clinical audit and research projects prioritised by the organisation Audit Strategy.

10.8. To act as a role model for all team members demonstrating and maintaining high standards of practice and professional conduct. Addressing issues of poor practice with senior line management and HR.

10.9. Co-ordination of team responsibilities ensuring equity of service provision and equity of workload within the District Nursing Team.

10.10. To have responsibility for a defined budget, by monitoring and controlling expenditure within their team by regular access to the portal system, liaising with their Management Accountant and Locality Manager.

10.11. To ensure a robust process is in place to maintain, monitor and authorise equipment and procurement of supplies for the Team.

10.12. Monitor and oversee duty rotas to ensure effective use of resources and adherence to the roster policy. Ensure rigorous control of bank and agency usage and the management of staff sickness and other leave whilst maintaining safe staffing levels.

10.13. Monitoring of sickness and staff performance.

10.14. To create, promote and maintain an environment conducive to team building, working and development.

10.15. To lead and support the team through change processes, being proactive with problem solving.

10.16. Providing support and promote clinical and line management supervision for all team members as per policies.

10.17. To ensure appraisal process within team at no less than yearly intervals, according to Organisation policy. To undertake appraisals as required ensuring line management responsibility throughout the team.

10.18. To represent the District Nursing Service at a strategic level.

10.19. To work with their team and other matrons to review staffing establishments and skill mix and workforce plans, and to ensure that staffing is appropriate for current and projected clinical demands.

10.20. To be identified Team Lead for Recruitment and Selection being aware of Organisation and employment processes and procedures.

10.21. To facilitate and support handling of complaints and conflicts within team, in accordance with the Organisation policies.

10.22. To be responsible for the ongoing monitoring and updating of the Electronic Staff Records.

10.23. To provide a direct link between the team and the Locality/Deputy Manager and liaise with Locality/Deputy manager regarding any areas of concern.

10.24. To ensure protective management time is included within the weekly work plan.

10.25. To plan, facilitate and monitor the professional leadership and managerial skills of caseload holders in the team to ensure they are working at the level stated in their job description.

10.26. To have a flexible approach to the working day in order to meet the needs of the patient and service.

10.27. To be aware of pressure facing your work colleagues and offer support and ensure they are aware of the organisation support services available to them.

10.28. To be team lead for risk register and escalation of risk across the team.

11. Communication:

11.1. To demonstrate clear understanding of the District Nursing Service, Primary Care, Organisation and Government objectives and communicate this to others.

11.2. Able to effectively communicate at all levels of the organisation, to a variety of health professionals, users and carers, to provide the best outcomes for people being cared for within the primary care setting.

11.3. To support, complete and monitor staff to ensure the submission of all necessary forms, documentation, including IT data and forward as required by the manager/organisation are undertaken in a timely manner.

11.4. Act as patient advocate as required to ensure individual needs, preferences and choices are understood and communicated to all involved in care delivery.

11.5. The post holder to be supplied with equipment necessary for their post for work purposes and be expected to abide by the Organisations policy.

12. Education & Training:

12.1. To facilitate and ensure equity of training for all team members, ensuring the team has the infrastructure to support induction, preceptorship, and professional development, pre-registration and medical students. To regularly review the skills escalator and career development of staff within the team in line with line management and appraisal.

12.2. Assume responsibility for own professional development and personal knowledge.

12.3. Monitor own performance against agreed objectives through the appraisal process, personal development plans and NMC regulations.

12.6. Plan and participate in the supervision, teaching and provision of community experience for all students and other personnel as appropriate.

12.7. Develop the learning environment to promote lifelong learning and staff development.
