The Trafford Renal Ward is a 26-bedded Ward for Nephrology patients of all backgrounds, acute and chronic offering nurses a chance to develop skills in highly specialised nursing. We work closely with the other areas of the Sussex Kidney Unit to provide specialist high quality care to patients with acute and chronic kidney diseases across Sussex.

You will help provide specialist high quality care to in-patients with acute and chronic kidney diseases. We are a very supportive team and can offer a structured Orientation and are happy to give you help and guidance for development. A kind and compassionate manner, and an interest in providing an excellent standard of care are essential.

Main duties of the job

• Deliver patient-centred nursing care on the renal ward, aiming to achieve the highest possible standards of care delivery, in accordance with the Trust's Strategy for Nursing and Midwifery.
• Working within the multidisciplinary team (MDT) to ensure high quality standards of evidence based care are maintained and Renal Association / Renal National Service Framework guidelines are met.
• In the absence of more senior nursing staff may be required to undertake specific ward management duties, as delegated by ward manager (e.g. taking charge of shifts).
• Provide best clinical and professional practice.
• Ensure efficient use of resources.
• Assist in the provision of a working environment which is conducive to learning for all staff and students.

Clinical

• Responsible for the assessment, planning and implementation of care and assists in the interpretation of data relevant to the patients condition and progress.
• Promote and implement evidence based practice including guidelines from relevant areas e.g. The Trust, The Renal Association, The Renal National Service Framework and The National Institute for Clinical Excellence.
• Maintain a working knowledge of preparing and implementing peritoneal dialysis, support and supervise HCAs skilled in the therapy.
• Negotiate effective nutrition / fluid intake with the patient in conjunction with other relevant members of the MDT.
• Enable patients and their families to understand renal failure and its treatment, and to gain access to further psycho-social support as required.
• Help and support a patient transferring between renal modalities and locations.
• Enable patients to make an informed choice regarding renal replacement therapies and conservative management.
• Build a partnership between the team, patients and carers and act as an advocate for the patient, relatives and staff.
• Ability and willingness to work towards a working knowledge of peritoneal dialysis, haemodialysis and plasmapheresis.
• Participate in the control, storage and competent administration of drugs and treatments as prescribed according to the NMC guidelines and the Trusts Guidelines for the Safe and Secure Handling of Medicines.
• Demonstrate skills in the care and management of the critically ill patient and work closely with all members of the care team to ensure the needs of the critically ill patient are met.
• Demonstrate proficiency in the care of CVP lines, and the administration of IV Therapy in accordance with Trust Policy.
• Participate in the smooth and effective discharge planning for patients.
• Maintain accurate documentation to facilitate the continuity of care for patients transferred to other areas.
• Promote a safe clinical environment for all patients, visitors and staff.
• Maintain the patients respect, dignity and privacy, and to act with kindness and courtesy to all members of the public.
• Be aware of essential procedures and take appropriate action in the event of cardiac arrest, fire or major incident.
• Actively participate as an effective team member, contributing to the efficient running of Trafford Ward including Coordinating the nursing care within allocated bay.
• Organise time, equipment and staff to deliver nursing care. Delegate and assign activities and tasks appropriately, working as a team to improve overall performance.
• Ensures stock levels are maintained and orders stock as required.
• Adhere to all local standard operating procedures and policies.
• Participate in co-ordinating the activities for the whole ward area managing staff placement, ensuring safe placement of patients in the ward area.
• Act as an advocate for the patient/client ensuring the provision of appropriate information and support services.
• Effectively liaise with ward staff, clinical operations department, site management team, discharge lounge and other departments to allow timely management of patient throughput.
• Ensure that discharge review documentation is completed and reimbursement material is communicated to relevant body in timely fashion.
• Participate in ward rounds and other multidisciplinary meetings ensuring that concerns of patients are identified and met.
• Ensure the health, safety and welfare of the ward/department staff, taking action where necessary and report issues of concern to the ward manager.
• Identifies incidences of concern to either patients or staff and deals effectively with the consequences, completing appropriate documentation (for example, Safe code reporting forms).
• Assists the Ward or Department Manager to develop and monitor systems to ensure co-ordinated and efficient patient movement including admissions, discharges and transfers in and out.
• Raise concern and escalate appropriately where unsatisfactory practice is identified.
