Sherwood Forest Hospitals NHS Foundation Trust is seeking a dedicated and proactive discharge specialist practitioner to join our team, focusing on discharge coordination. The successful candidate will play a pivotal role in ensuring the timely and effective discharge of patients across the Trust. Working closely with ward-based multi-disciplinary teams and the discharge hub, you will:
  • Ensure the provision of specialist advice.
  • Develop and implement policies to manage the discharge process.
  • Ensure all patients with long lengths of stay over 14 days have clear discharge plans.
  • Conduct Reason to Reside checks for all patients.
  • Participate in the Length of Stay (LOS) report weekly meetings and other relevant meetings.

Knowledge Requirements

Essential

  • Excellent communication and time management skills.
  • Ability to change, manage and motivate others
  • Knowledge of audit and research


QualificationsAcademic/ Craft/ Professional

Essential

  • First level registered nurse or AHP
  • First degree
  • Teaching qualification


Desirable

  • Masters degree
  • Community experience
  • Experience in HCOP


Further Training

Essential

  • Mandatory update
  • Management course (or working towards)
  • Car driver
  • Computer skills


Desirable

  • ECDL


Experience

Essential

  • Experience at a Band 5 or above with specialist skills and knowledge in assessing the needs of patients with complex health and social care needs and extensive experience within medical / care of the elderly nursing.
  • Knowledge and experience of intermediate care services.
  • Evidence of multidisciplinary working at an advanced level.


Desirable

  • Knowledge of the legislation for discharge processes
  • In depth knowledge of continuing health and social care policies
  • Policy writing and interpretations
  • Change management skills
  • Recent participation in clinical audit and / or research project
