We are recruiting a registered Nurse for our Witherow ward at St Mary's Hospital Paddington. It is a 12 bedded ward which specialises in male elderly care medicine. Patients with an average length of stay of 12 days or less. We offer an environment that will enable you to develop and broaden your nursing portfolio including caring for patients with a variety of medical conditions and an in-depth understanding of frailty and hospitalised deconditioning.

You will work as part of the Care of Medicine for the Elderly Team supporting the division and Manager of the Witherow ward to ensure an effective and high quality of care and service is delivered to those patients who access and requires treatment daily.

If you are a Registered Adult Nurse, with experience at Band 5/6 or above in an specialist care of the elderly hospital setting and are innovative, driven and passionate about delivering excellent, person-centred care then this job could be for you.

Main duties of the job

  • You will be 85% Clinical based and 10% managerial. This means that a full time post will require you to work at least 1 day per rota in a managerial role.
  • You will be required to promote the inter-professional working relationship within the service - administrative, consultants, physiotherapist, dietician specialist nurses, the discharge hub, occupational therapist, religious leaders, specialist teams such as the dementia team, etc.
  • You will be responsible for the professional day-to-day clinical leadership and operational management of Medicine for the Elderly as the Nurse in Charge. Ensuring high quality, responsive, safe, efficient and compassionate care is delivered and professionalism is maintained amongst staff.
  • You will support the Ward Manager in developing quality improvement projects that address frailty, hospitalised deconditioning and reducing long length of stay.

Person Specification

Education/ Qualifications

Essential

  • Active Registration with the NMC
  • Degree/diploma in health related subject.


Desirable

  • Evidence of appropriate post graduate education/CPD
  • Mentorship qualification.


Experience

Essential

  • Substantial post registration experience
  • Evidence of working as an autonomous practitioner.
  • Evidence of close working with all members of the multidisciplinary team in a medicine for the elderly ward.
  • Aware of recent developments within nursing. Particularly care of the elderly.
  • Understanding of clinical governance.
  • Awareness of national guidelines and standards of care of the elderly.


Desirable

  • Proven experience in a care of the elderly specialism.
  • Experience of implementing quality improvement projects, based on evidence based practice.
  • To have had experience in clinical audit, research or policy/guideline development
  • Experience of contributing to care pathways and discharge process of the care of the elderly.


Experience Skills/Knowledge/ Abilities

Essential

  • Substantial post registration experience. Evidence of working as a Band 5.
  • Evidence of close working with all members of the multidisciplinary team. Proven knowledge of Anatomy and Physiology of ageing. Aware of recent developments within nursing. Understanding of clinical governance. Awareness of national guidelines and standards of care on the management of elderly patients. Knowledge and understanding of measuring outcomes
