We are looking for innovative and enthusiastic Mental Health Nurse to join and complement our existing experienced CMHTOP in North East Hants.
You will be a core member of the Multi-Disciplinary Team based at Aldershot Centre for Health, participating in the provision of on-going assessments, care planning, treatment and support of people with serious and enduring mental health problems and people in crisis to promote health, independence and recovery.
You will have:
- Current full UK NMC registration
- Two years post qualification working in the community with people with severe and enduring mental health illness
- Current UK driving license and access to a vehicle

Main duties of the job
- To deliver a high-quality service to older people with mental health diagnoses and/or dementia, in a multi-professional framework.
- To develop shared understandings of people's presentations that help people to better comprehend the underlying function(s) driving behaviour that challenges.
- To act as key worker/care co-ordinator for named individuals and adhering to the Care Programme Approach process.
- The ability to work independently, autonomously and take personal responsibility/initiative.
- To provide high standards of care, which are delivered in a professional manner.
- To ensure that the care delivered is based on best practice and is regularly reviewed.
- To support the team on the duty rota, screening team referrals and triaging accordingly. Responding to those that are urgent.
- To ensure all records and reports are kept up-to-date and are in accordance with Trust electronic records system guidance and key policies such as the Caldicott report.
