An exciting opportunity has arisen to work with the Rapid Community Discharge Service based at RBH.

We are looking to recruit a band 7 discharge facilitator for the Rapid Community Discharge.

You will be working as part of a small team who work across 7 days a week Monday - Sunday to support and enable the effective flow of patients from the acute hospital ensuring successful and timely onward transition to the next step in the patient's pathway.

This role entails working with multi-systems partners to aid patients' safe discharges from the acute unit to various community settings. You will be working closely with the organisation, integrated teams, key stakeholders, and partners.

You will be responsible for helping in developing junior staff, managing the digital medically optimised caseload with support of the Team Leads.

Excellent communication is a key skill required for the role you will have previous experience of working in the NHS. You will have excellent organizational and computer skills and be able to demonstrate an ability to navigate various systems and software packages.

Main duties of the job

The post holder will work as a Senior Discharge Facilitator for the Rapid Community Discharge Team based at RBH.

The postholder will need to work closely with key stakeholders within the Acute Trust - delivering a community driven, discharge to assess approach to discharge - enabling the effective flow of patients into the right service at the right time.

The team will work to ensure a seamless interface between RBH colleagues identifying patients medical fit for discharge, and then ready to leave hospital.

As discharge facilitator you will ensure successful and timely onward transition to the next step in a patients pathway - this will include liaising with ward based OT's, flow coordinators assigned to in - patient wards and complex case managers working on the early identification of patient prior to becoming medically fit . Facilitators are expected to work autonomously to manage patient discharges on every shift - taking appropriate actions to achieve a safe and timely discharge.
  • Whilst on shift the postholder will assist with all administrative functions to support effective discharge of patients
  • Maintain all office systems both electronic and hard copy across the service.

Assist in the efficient and accurate recording of data on electronic systems (RIO, Teams ESR Agresso, e- Manager)

Essential

  • Evidence of post graduate study
  • Evidence of continuing professional development


Desirable

  • Registered clinician/social worker/ OR educated to degree level.

Previous Experience

Essential

  • Extensive experience of working in a community services setting
  • Previous experience of multi-agency collaboration working across professional boundaries
  • Ability to effectively challenge stakeholders while maintaining effective relationships
  • Knowledge of hospital discharge process


Desirable

  • Experience of managing people and teams including performance management, sickness monitoring, recruitment, skill mix and staffing rotas
