This is a really exciting opportunity for a Band 6 Registered Mental Nurse or Social Worker to join our Older Adults Community Mental Health Teambased in Sale at Crossgate House, which provides a peripatetic service topeople living across the borough of Trafford.
Main duties of the job
The Older Adults Community Mental Health Team in Trafford is an integrated health and social care service and strives to provide quality recovery-based support to people with organic and functional mental health problems.
Under the line management and clinical supervision from within the CMHT the post holder will coordinate CMHT duty service Monday to Friday 9am - 5pm.
This involves management and triage of referrals into service, planning andproviding timely assessments and interventions and responding to duty calls from referrers, service users, carer and other providers. The post holder will clinically manage the referrals and assessments in line with the clinical pathways within the CMHT operational procedure.
This includes respecting the individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs

To demonstrate high-level communication skills.
To utilise highly developed interpersonal skills including specific skills to facilitate effective communication when there are barriers to understanding e.g. sensory or cognitive impairment or in a hostile, antagonistic or highly emotive situation.
To establish and maintain robust communication networks with clients, carers, and other health care workers and numerous external agencies. To establish and maintain good working relationships with GPs.
To promote awareness of the role of CMHT within local service area/team, GP practices and the wider community.
Be a full member of the multi-disciplinary team (MDT).Accept referrals from agreed agencies in accordance with agreed operational procedures. Effectively manage own caseload.
Undertake assessments of clients as part of the CMHT duty system in accordance with agreed operational procedures.
Assess, care-plan, provide interventions and evaluate and record community mental health nursing delivered to clients on own caseload. Provide specialist mental health nursing assessments as required in line with operational procedures.
To promote positive mental health and physical care to clients in all situations.
To encourage and support clients to collaborate in their treatment programme and recovery.
To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.
To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
To undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self-harm or risk of physical, sexual or emotional harm to others, using the agreed Risk Assessment Tool.
Administer medications as required. E.g. depot injections.

Person Specification Education/Qualification Essential
- Education/Qualification - Registered mental Nurse compliant with requirements for NMC registration.
- Education / Qualifications - Education and Mentorship Module ( or willingness to undertake)
- oExperience of working within a multi disciplinary team.
- oKnowledge of requirements under the Care Programme Approach.
- oEffectively manage own caseload.
- oAbility to conduct first line assessments for individuals newly referred into the service, utilizing a biopsychosocial framework.
- oFull understanding of the Care Programme Approach and Mental Health Act including the needs and rights of Carers.
- oGood Knowledge and application of Risk Management.
- oGood Knowledge off Adult Safeguarding Procedures.
- oGood knowledge of Mental Health Act and Mental Capacity Act.
- oUnderstanding of current national policy and initiatives in relation to enduring mental health issues.
- oAbility to establish good working relationships with a range of providers including GP's.
- oMaintain confidential case records.
- oParticipate in multi-disciplinary clinical meetings.
- oAct as a Mentor or supervisor to learner students.
- oCar driver / owner.
- oAbility to assess referrals using the CPA framework.
