Tobe a core member of the multi-disciplinary/multi-agency service and consulting, participating, or leading in the provision of crisis and risk assessments, planning care, and bringing together those teams/agencies to support CYP in their recovery journey. Often this response will be to facilitate a diversion from T4 admission or out of area placement and allow for recovery as the C/YP remains in their home/placement/community/school. There will be a role in connecting and communicating with T4 providers should C/YP be admitted for an inpatient stay and assisting with discharge planning.
Main duties of the job
Report to the General Manager (GM) and Clinical Nurse Lead and contribute to the development of the health pathway and delivery of service.
To provide timely assessment of CYP who are referred and who may require diversion from T4 admission or out of area placement or who need T4 admission or a placement to meet their biopsychosocial needs.
To provide support, advice and education to the Acute Hospitals, and other emergency service provision such as the Police to build knowledge and skills of staff within these settings.
To work individually and jointly within the Mindworks alliance to provide a specialist, assessment, and treatment service to CYP and their families/carers in crisis.

Job responsibilities
Band 5
Work closely within the Mindworks alliance and develop understanding of the ithrive model and how the Mindworks Alliance collaborate.
Undertake clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis under supervision, working alongside medical, nursing, and other professional colleagues to ensure that the most effective care and treatment is provided to meet their current needs.
Work closely with the Emergency Departments, Crisis Intervention Service, Extended Hope, CCISS, the Adult Psychiatric Liaison Service, other Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, Paediatric Liaison Nurses, ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed
Develop/update the My Safety Plan with C/YP and families to ensure a robust crisis and contingency plan.
Work within a highly experienced multi-disciplinary team involved in the assessment and treatment of CYP with serious and complex mental health difficulties in crisis.
Offer support for Students on placement.
Co-work with other members of the integrated team.
Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies, legal requirements and the NMC Code of Conduct.
Have basic IT skills and data entry skills to enter clinical information on the databases.
Contribute to the teams overall quality standard of services provided.
Through the appraisal and supervision systems help understand/identify own training needs and undertake continuous professional development.
Ensure high standards of care are provided, and that regard to dignity, individuality, self-esteem and wellbeing of individuals and their families are given. Monitored and evaluated on a continuous basis.
Ensure that outcome measures for the Hope Service are implemented.
Interface with other services/agencies regarding care, treatment, and discharge planning.
Through supervision be supported towards developing into the Band 6 role of Mental Health Nurse Practitioner using the competency framework as a guide.
Band 6 (in addition to Band 5)
You will lead clinical assessments of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing, and other professional colleagues to ensure that the most effective care and treatment is provided resulting in safe, timely discharge
Co-ordinate and liaise with Mindworks Alliance responding to crisis and urgent referrals of CYP/ or for crisis and urgent presentations of CYP already working with the Hope and Ext Hope Services.
To work as an autonomous practitioner with your own defined caseload and contribute to the work of the integrated team.
Ensure you engage with supervision, learning sets and other forums as part of your ongoing learning and development.
Contribute to audits of outcome measures for the service and to any relevant research/evidence-based practice that can inform developments within the service.
Be responsible for the initial assessment and discharge/ or care coordination role for the C/TP utilising fully the electronic patient record (EPR)
Advise referrers on outcome of assessment and respond to requests for and provide formal reports for other agencies.
Represent the Service at professional/network meetings to offer advice/consultation.
To undertake relevant CPD to improve assessment and treatment skills regarding crisis care.
Lead on transfer of C/YP from Hope Service back to other CYPS Services or transitions to Adult services.
Development Expectations: Overview (Example only will be personalised to meet your needs once in post).
You will be allocated a mentor to support and coach during the first 6 months, usually the Clinical Nurse Lead. The following is an outline of the development expectations over time for the post. However, it is not exhaustive, and the pace will be dependent on your own developmental pace.
The clinical and managerial supervision process within the service will monitor and tailor the process to each individual and will monitor progress after the initial 6 months.
It is expected that within 12-24 months of the development period you will be able to demonstrate meeting the required competencies at a Band 6 level as outlined in the above competency profile and that you will raise with your mentor if there are any difficulties or support needed. The decision of progressing to the Band 6 role will be discussed by your mentor and the GM and agreed.
