CLCH Hertfordshire Adult Community Services -CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

West Hertfordshire community respiratory service provided by CLCH is expanding and looking for an enthusiastic forward thinking respiratory nurse specialist .

We are a large, friendly, supportive multi-disciplinary team and are based in Hemel Hempstead providing care for patients in Herts Valley CCG. We work predominantly in the community with patients who have long term respiratory condition COPD, Asthma, Bronchiectasis, ILD and OSA.

The service is undergoing exciting integration with our Acute Care colleagues at West Herts Hospitals Trust. As such existing pathways and new areas are being reviewed and developed, opening up new opportunities for staff to work in clinical areas like 7 day Airways Virtual Hospital, attend specialty MDTs, complex NIV and Sleep, as well as continue providing Nurse/Physio specialty clinics, HOSAR, amongst others.

Working effectively within a multi-disciplinary team and demonstrating effective communication skills is essential. Car owner and driver essential

Main duties of the job

To work within a multidisciplinary respiratory team to deliver community respiratory service in West Hertfordshire providing specialist services including Airways Virtual Hospital (admission avoidance and early supported discharge), home oxygen assessment & review service and nurse led clinics and home visits to provide expert specialist care to patients with COPD, asthma, bronchiectasis, OSA and ILD.

Job description

Job responsibilities

To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are evidence based and which enhance the quality of patient care.

To undertake comprehensive assessments of complex respiratory patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.

To assesses, develop and implement specialist care programmes, including home oxygen therapy, home NIV and CPAP, pulmonary rehabilitation, psychological interventions and medication reviews, adhering to evidence based guidelines.

To accurately undertake specialist clinical skills including performing arterial / arterialised blood gases and spirometry, venepuncture.

Person Specification

Education/Qualification

Essential

  • RN Level 1 registration with the NMC
  • Post registration respiratory qualification or equivalent experience
  • Teaching/Mentorship qualification or equivalent experience
  • Possession of a degree or equivalent


Experience

  • Experience of working with respiratory patients
  • Experience of working within a community setting.
  • Demonstrates ability to develop new initiatives.
  • Demonstrates ability to managing change
