An exciting opportunity has arisen for an experienced nurse to join the Heart Failure Specialist Nursing Team in South Tyneside and Sunderland NHS.
You will need to have a sound knowledge of cardiology nursing with a desire to develop skill and knowledge applicable to heart failure.
You will work autonomously and have responsibility for proactively managing a caseload of heart failure patients in a Primary and Secondary care setting.
We will offer you pathway of progression, training, and career structure and this post will develop to a Band 7 upon completion of developing practice in clinical skills module and independent Non-Medical Prescribing qualification.

Main duties of the job
We are part of the cardiothoracic directorate, and you will work within a team of heart failure specialist nurses and the wider cardiology team based at South Tyneside and Sunderland Foundation Trust.
Provide comprehensive, effective and specialist nursing care and advice to patients, carers and health care professionals.
Assess, plan, develop and evaluate programmes of patient care in collaboration with the Heart Failure Specialist Nursing team.
Provide pharmacological management of evidence-based therapies to reduce mortality and improve life expectancy in line with national policy.
It is essential you have a full UK driving licence and access to a car.

- Work as part of the multidisciplinary team working under the direction of line manager/Matron.
- Continue to develop advanced knowledge and skills within the specialist sphere of practice.
- Support with the planning, delivery and evaluation of care pathways for a designated group of patients under the direction of the line manager/Matron.
