If the answer is yes then an exciting new challenge awaits with Gloucestershire's new Integrated Urgent Care Service, within the Out of Hours area.

We have an amazing opportunity for a trainee band 7 Advanced Practitioner to join the Out Of Hours Service.

Advanced Clinical Practice should be delivered by experienced registered health and care practitioners. It is a level of practice characterised by a high level of autonomy and complex decision making. This is underpinned by a Masters level award or equivalent that encompasses the four pillars of practice:
  • Clinical practice
  • Leadership and management
  • Research
  • Education

What we are looking for:
  • Relevant courses at level 7 or equivalent in Advanced Physical Assessment skills, Minor Illness, disease management, etc.
  • A non-medical prescribing qualification (desirable).
  • Evidence of continued professional development.
  • Satisfactory DBS clearance.
  • Experience of managing acute undifferentiated presentations.
  • Ability to triage.
  • Good understanding of autonomous practice within community services including prevention of hospital admission.


Main duties of the job

In addition to standard nursing/ therapeutic duties, the post holders will be:
  • Accountable for clinical governance and leadership within the service.
  • Expected to work across organisational and professional boundaries.
  • Develop effective partnerships with stakeholders.
  • Implementing evidence-based practice is inherent throughout all aspects of care delivery and treatment, to ensure this knowledge is disseminated throughout the service.

Practice at a high level in assessing and diagnosing patients with a range of clinical presentations within the scope of community urgent, and OOH care.

Qualifications

Essential

  • Professional registration with registered body e.g. NMC, HCPC, GPhC with significant post-registration experience relevant to role
  • MSc Advanced Practice and NHSE Digital Badge or evidence to support Professional clinical knowledge that covers all 4 pillars (as above) of the Advanced Clinical Practice capabilities (including physical assessment and physiological reasoning qualifications at Masters level) and ability to undertake supported e-Portfolio Route to gain NHSE Digital Badge
  • Recognised teaching / supervisor or coaching qualification or relevant experience
  • Registered as an independent prescriber with the relevant registered body (NMC, HCPC, GPhC etc)


Desirable

  • Qualification in verification of an expected death


Experience

Essential

  • Skills of assessment, examination, diagnosis and treatment within an agreed scope of practice, to include newborns, paediatrics and end of life care
  • Significant post registration (the multi-rofessional framework for advanced clinical practice in England suggests 5 years) experience where autonomous working at an advanced level has been acquired in the speciality area or a related area where advanced skills could be transferable
  • Managerial and clinical leadership experience within a large multidisciplinary organisation Proven track record of teaching and assessing.
  • Experience working in Urgent/ Primary/ Acute care in a community setting


Desirable

  • Experience in chronic disease management
  • Experience in remote consultation
  • Experience of work using telephone triage
