Shropshire Community Health NHS Trust has exciting opportunities for Band 5 Community Staff Nurses to join our Community Nursing Team's across all areas of Shropshire on a bank contract.

We are a proactive and forward thinking teams, who constantly strive for the delivery of excellent patient care and modern ways of working. The team's work closely with Therapy, Virtual Ward and Rapid Response supporting multi professional working.
The services operate between the hours of 8am-6pm, 7 days a week, 365 days a year. As this is a community post, the successful candidates will need to be able to meet the travel requirements of the post please demonstrate on your application form how you would meet this requirement.

Main duties of the job
You will need to be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development, we offer a nurturing environment allowing you to reach your full potential.
The team's provide nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
You need to be an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team.
You will be will be accountable for the delivery of high quality care to patients whilst working with other members of the multi diciplinary teams to promote independence for patients on the team caseload.
