The specialist Occupational Therapist will work in Elmhurst - Discharge to assess unit based in Winsford. Elmhurst is a 30 bedded unit providing round the clock nursing support to adult patients who are in transition from hospital to community. They will be working closely with the MDT and provide supervision to the less experienced therapy staff and assistants.

Main duties of the job

To be responsible for providing Specialist assessment and treatment to patients with diverse health needs in discharge to assess unit. Promote safe and timely discharge of patient to their home after a period of therapy. Ensuring appropriate pathways of care and communications via liaison and referral to other agencies as required.

Job responsibilities

SERVICE DELIVERY
  • To contribute to the effective day to day provision of Occupational Therapy in your working area, reporting to and liaising with the team leader and manager.
  • To manage own complex case load of referred patients. Monitoring the appropriateness of referrals, obtaining information relevant to patient care and ensuring appropriate discharge/ongoing care.
  • To develop effective systems of communication and liaison with referring agents and all members of the multi-disciplinary team. Working across boundaries with partner and voluntary organisations. Providing and receiving highly complex information, sometimes of a sensitive nature, using developed interpersonal and communication skills where co-operation is required and there may be barriers to communication.

  • GOVERNANCE
  • To maintain an awareness of relevant new and current professional, Trust and developmental standards, policies, procedures and guidelines, ensuring compliance.
  • To support the implementation of national & professional guidelines e.g. NICE
  • Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care.

  • MANAGERIAL/LEADERSHIP
  • Ensure To contribute to the development objectives of the Care Community Service via involvement in meeting the service development plan actions.

  • 2. To respond to the needs of professional clinical audit and research within the Trust and Service, and to participate actively as required.
  • Ensure a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.
  • To record and submit data as required by the Trust, Care Community Service Manager and Therapy Manager in an accurate and timely manner.
  • To monitor equipment used, notifying the Team Leader of any malfunctions and adhering to equipment servicing/loan procedure.

  • EDUCATION/LEARNING
  • To attend staff, peer review and in-service training meetings.
  • Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.
  • Clinically supervise newly qualified Physiotherapists and physiotherapy assistants and undertake KSF and junior objective appraisal setting as required.
  • To act as a clinical educator and contribute to the education and training of Occupational Therapy students to graduate level.
