This project forms part of a wider plan to improve urgent and emergency healthcare delivery.
We are committed to revolutionising healthcare delivery for frail and elderly patients by launching a Community Frailty Virtual Ward. Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions and length of stay in hospital and enhancing overall well-being through enhanced care coordination.
Interviewing between 9-15th September at the Beauchamp Centre, Mount Gould Hospital. Please bring with you:
Copies of any qualifications relevant to the role
Copies of training certificates (digital copies are acceptable)
3 forms of identification, to include your Right to Work documentation and 2 proof of address (ideally a printed bank statement from a Current Account). This must also include at least one photographic identification document. This will allow us to start pre-employment checks without delay.
Main duties of the job
As Dietician, you will play a critical role in our new multidisciplinary team, the 6 new frailty virtual wards that we have planned for our locality. The post holder will work closely with consultants, pharmacists, ACPs and other healthcare professionals to ensure seamless, integrated care on the virtual ward, by providing specialist ACP assessments and interventions for those admitted to the Frailty Virtual Ward.
Key Responsibilities
To provide a specialist, high-quality nutrition and dietetic service within Frailty Virtual Wards.
To provide specialist dietetic assessment, diagnosis, treatment and education to individual patients with on the Frailty Virtual Ward.
To provide advice, signposting, guidance and information to health and social care professionals, patients, carers, relatives or other non-professional contacts
To be an integral member of the MDT providing education, training and dissemination of information to all healthcare professionals.
To be an autonomous practitioner, holding a clinical caseload and working without direct supervision
To continue own professional development in line with HCPC regulations
A secondment will only be considered if your current manager is supportive of you undertaking a secondment. A secondment may have the possibility of becoming permanent

- Competence with patients with complex needs, communication issues and/or challenging behaviour.
- Varied and comprehensive post-graduate clinical experience across a range of clinical areas.
- Experience of working in a multi-disciplinary team.
- Experience of delivering in-service training.
- Experience of interpreting and acting on complex clinical information including anthropometry, biochemistry, pharmacology and medical history to make decisions about client care.
- Evidence of participation in audit activities in dietetic practice.
- Experience and competence in negotiating and recording aims of treatment and agreeing a plan of action with client/relatives/patients/medical staff.
Desirable
- Competence and experience in advanced communication skills including negotiation, motivational interviewing and evoking behavioural change in complex cases.
- Knowledge and Experience of enteral feeding and re-feeding syndrome as well as alternative methods of nutritional support.

- Evidence of further formal study leading to postgraduate qualifications e.g. Diploma in Advanced Dietetic Practice, Masters degree.
- Working towards or hold a certificate in counselling skills or have extensive counselling experience.
- PENG clinical update.
- A teaching qualification or experience of teaching groups.
- Clinical Supervisory Skills Course.
