An exciting opportunity has arisen for a Band 6 Occupational Therapist to work within the our friendly and supportive Calderdale Community Rehabilitation Teams on a permanent basis, up to 37.5 hours/week available.
The Community Rehabilitation Team is a multi-disciplinary team, and consists of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Dietetics, Falls Prevention Workers and others support staff. The different disciplines work together in an integrated manner to provide the best treatment for patients with a range of acute and long-term conditions.
The majority of clinical work takes place within patient's homes (including care homes). However, we also see patients in clinics or groups when it is more appropriate to do so. At times, the team visits patients in hospital to participate in meetings, or to assist complex discharge planning. Therefore you will require access to transport with business insurance.
Main duties of the job
To have specialist assessment and treatment skills in the areas listed but not limited to:
Determine a clinical diagnosis, develop, deliver, implement, evaluate and modify individualised treatment programmes and/or provide specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required

Job description
Job responsibilities
- To develop specialist Occupational Therapy interventions in line with current evidence base.
- To manage autonomously a clinical workload of patients / clients at a specialist level, assessing and treating patients and maintaining clinical records.
- To apply evidence based knowledge within the specialist clinical area acquired through experience and academic study to a specialist post graduate level.
- To set SMART goals to ensure treatment is effective, and to improve patient flow through the service.
- To work collaboratively with health and social care community and acute colleagues.
- To work with other members of the MDT to ensure effective care pathways and transfer of care / discharges.

Essential
- Degree/Diploma/MSc in Occupational Therapy
- HCPC Registered
- Committed to undertaking further training to develop practice, including HEI and M level study
- Trained Clinical educator
Desirable
- Masters level qualifications in specialist area
- Skill-sharing/blurred boundary competencies
KNOWLEDGE, EXPERIENCE & EXPERTISE
Essential
- Knowledge of relevant legislation, national guidelines and best-practice for a range of conditions
- Comprehensive knowledge of appropriate and available Occupational therapy assessments and approaches, including outcome measures and goal setting
- Problems solving/decision making skills based on patient/client needs and goals
- Recent experience demonstrating ability to assess, treat and evaluate care at a specialist level in rehabilitation and Long Term Conditions
- Experience developing, supervising, appraising and training others
- Experience of diffusing conflict and complaints
- Proven ability in implementing Clinical Governance

- Ability to case manage patients with long term conditions/multi pathologies
