An exciting opportunity has arisen with the IPBT Lincoln South for a Band 6 Mental Health Practitioner.
Are you are a registered healthcare professional and passionate about providing excellent outcome focused quality driven care. Do you want to influence and work within exciting and transformed services that value new approaches for working in partnership with others in the delivery of ‘joined up’ care for our communities across Lincolnshire. If this is you, and you are ready for a new challenge, we would really love to hear from you.
Following a successful bid for transformation funding, Lincolnshire has been chosen as one of twelve pilots to develop new place based integrated care pathways for individuals with severe and complex mental illness through a neighborhood working approach.
You will work autonomously in these roles in the context of local and national drivers providing advanced clinical care, using high level assessment skills for identification of individual needs which essentially supports diagnosis and treatment options. You will forward or signpost individuals to the most suitable service dependent on your assessment and crucially co-ordinate the patient’s journey across the health, social care system, including voluntary and third sector partners, regardless of care setting.
