Following a successful bid for transformation funding, Lincolnshire has been chosen as one of twelve pilots to develop new place based integrated care pathways for individuals with severe and complex mental illness through a neighborhood working approach.

You will work autonomously in these roles in the context of local and national drivers providing advanced clinical care, using high level assessment skills for identification of individual needs which essentially supports diagnosis and treatment options. You will forward or signpost individuals to the most suitable service dependent on your assessment and crucially co-ordinate the patient’s journey across the health, social care system, including voluntary and third sector partners, regardless of care setting.
