An exciting opportunity for qualified nurses, OT's, social workers and Clinical/Counselling psychologists to work at the heart of primary care delivering mental health support in conjunction with mental health services.

North Devon Coastal PCN (northerndevoncoastal.co.uk) and Taw Adult Core Mental Health Team are excited to be recruiting a Band 7 Specialist Clinical Practitioner.

The teams are building robust relationship as a commitment to improving the mental wellbeing of its population as part of an ambitious programme of implementation of the Community Mental Health Framework.

Main Duties of the Job:
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients presenting with undifferentiated diagnosis and with complex needs including long term conditions
  • Prioritise mental health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care and well-being

About you
You will be working within the primary care network bringing specialist mental health knowledge and skills into this community space, applicants will have significant experience of managing risk, have proven skill in delivering psychologically informed interventions and a working knowledge of primary and secondary mental health care services.

You will be empowered to support patients with complex conditions, adopting a personalised approach to patient care that maximises independence, choice, and control, following a philosophy of enabling people to reach and maintain their optimum level of health and wellbeing.

You will not be working alone but as part of a Multi-Agency / Multi-disciplinary service with a strong connection to the Core Mental Health Service, VCSE provision and primary care colleagues covering this PCN neighbourhood.

An ability to travel to meet the requirements of this role will be essential.

Your base will be West Pilton Health & Wellbeing Clinic, Youings Drive, Barnstaple EX31 1QL,however under the current COVID-19 conditions this may be subject to alternative arrangements/change for example home working.
