Clayponds Hospital is an in-patient rehabilitation unit comprising of three wards Jasmin, Magnolia and Rosemary ward. Rosemary ward is our stroke and neuro unit.
The Hospital provides care and intervention to patient with various neurological, musculoskeletal and other medical conditions following stabilisation of their acute medical issues.
The neuro rehabilitation service is provided by a multidisciplinary team specialised in neurology, including doctors, nurses, therapists, and health care facilitator.
The team work in collaboration with patients in meeting their rehabilitation goals up to 8 weeks.
As a Registered Nurse you will be responsible for supporting your Manager in ensuring that our service users' needs are met through the delivery of high-quality care. As a centre of excellence, you will offer a person-centred approach to their independence and well-being.
This job description is intended as an outline indicator of general areas of activity and will be amended in the light of the changing needs of the organisation. To be reviewed in conjunction with the post holder on a regular basis. You may be required to work at other locations within the Trust as determined by the duties of your post. You may be required to undertake any other duties at the request of the line manager which are commensurate with the role, including project work, internal job rotation and absence cover.
Main duties of the job
Maintain a high standard of clinical work when carrying out nursing procedures.
Responsible for the safe custody of medication, including checking and administration.
Ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement.
Nurse in charge of allocated patients and health care facilitators.
Provide basic nursing care to advance nursing procedures such as drug administration, wound dressing, catheter care & management, care & management of urinary and bowel management, care & management of PEG/RIG Feeding, safe & effective drug administration, etc.
Completing referrals for TVN specialist, Bladder and/or Bowel Continence, District Nurse, Social Services such as Sec 2 Admission Notification and Sec 5 Discharge Notification, etc.
Ensuring referrals are made to appropriate multidisciplinary team such as dietician, SLT, neuro psych/Psych liaison.
Provide assistance and support to MDT as well as family and/or main carer.
Provide supervision and support to health care facilitator as well as student nurse and apprentice nurse on the ward.
Communicate to patient's family and/or main carer for any updates about the patients.
Represent nursing team to MDT meeting, ward round, family meeting, best interest/professional meeting, etc.
Support the ward in completing of Clinical Audits
