A vacancy has arisen within CIS Stafford for an enthusiastic individual to join our Nursing Team, providing care to clients in their own homes.
You should have excellent communication and interpersonal skills and the ability to organise and prioritise your workload without direct supervision. Along with other Band 6 colleagues, you will support the Operational Lead and Clinical Lead in managing and coordinating aspects of care and service provision including case management within the team.
Past experience within a community setting is essential. You must have excellent leadership skills and possess the Special Practitioner Qualification. You will be supported by the Clinical Lead and Operational Lead and through peer support from other Band 6 District Nurses within the team. There will opportunities for personal development through informal and formal learning.
Main duties of the job
The Community Intervention Service provides care to patients in their own homes and is pivotal in the prevention agenda. This includes supporting the care of frail older people with long term conditions as well as acutely unwell patients in the community.
As a community staff nurse you will deliver a high quality service, working closely with GP's and a multidisciplinary teams in addressing a range of care issues across a spectrum of health conditions and preventing avoidable hospital admissions.
We are an innovative service covering Stafford and surrounding areas with a number of local GP Practices and are seeking enthusiastic nurses to work within our team.
You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care adhering to policies and procedures.
You must be a qualified nursing practitioner with current NMC registration, with experience of working in the community you need to be able to demonstrate a commitment to delivering high quality targeted personal care and hold a full driving licence.
