This is an exciting new post within Swansea Bay CAMHS based at Neath Port Talbot Hospital.
The successful person will be responsible for planning, managing, coordination and reviewing the care of children and young people eligible for continuing health care and jointly funded care packages with complex needs within a defined caseload.
Main duties of the job
Assessing children and young people with complex health needs which include learning disability and/or mental health for continuing care needs.
Be responsible for planning, managing, coordination and reviewing the care of children and young people eligible for continuing health care and jointly funded care packages with complex needs within a defined caseload.
The care coordinator will ensure holistic care is delivered in a timely manner, coordinating all agencies in providing both practical and emotional support.
To work within agreed timescales as recommended within the National Framework for Children and Young Peoples Continuing Care (2016).
Work collaboratively across primary, secondary, social care for the designated patient group.
To ensure the continuing care framework is followed and that care packages are clinically appropriate.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
