We are looking for a skilled, motivated and enthusiastic Band 5 Speech and Language Therapist who would like to join our dynamic multidisciplinary team in Battersea.
The Wandsworth Maximising Independence Team is a service for adults within Wandsworth who require rehabilitation in the community. The service is part of Wandsworth Community Health Services, and prides itself on its integrated working. We work alongside our nursing colleagues, GPs and specialist community services to provide excellent community based care to patients within Wandsworth. We work closely with our partners in local acute care providers, social care, and third sector to better meet complex patients' needs.
The team comprises of a large supportive multidisciplinary team physiotherapists, occupational therapists, speech and language therapists, and rehab support workers. It provides an urgent and routine pathway to adults in the community, working with them and their families or carers to help regain their independence following illness, preventing avoidable attendance or readmission to hospital, supporting them to manage long term conditions and promoting health and wellbeing.
This post offers the opportunity to develop your clinical skills working with a predominantly dysphagia based caseload. You will gain experience working with patients with conditions such as dementia, COPD, Parkinson's Disease, stroke and other long term conditions.

Main duties of the job
- Ability to demonstrateworking autonomously as a Speech and Language Therapist in developing treatment plans, assessment and discharge planning for patients.
- Providing dysphagia and communication intervention for patients in an array of community settings e.g. their own homes, care homes and supported living settings.
- A positive attitude and commitment to providing excellent holistic care and evidence-based interventions.
- Demonstrating ability to work proactively as part of a multidisciplinary team.

- Recognised Speech and Language Therapy degree
- Registration with Health & Care Professionals Council
- Part completion or completion of dysphagia competencies
Desirable
- Evidence of relevant post-registration education and training e.g. in Stroke, Parkinson's, Dementia
- Member of a relevant Clinical Excellence Network
- Registered member of RCSLT
- Completion of NQPs
Experience
Essential
- Experience of working with an adult caseload relevant to the post.
- Experience of working and liaising with other MDT professionals.
- Experience of providing assessment and intervention with adults with communication and swallowing difficulties.
Desirable
- Experience of working within a community SLT service, including in care homes.
- Participation in audit research projects service development.
- Experience of delivering training.
- Experience in working with end of life patients.
Knowledge
Essential
- Knowledge of assessment tools relevant to an adult client group including: dysphagia, aphasia, dysarthria, dyspraxia, dysphonia, cognitive communication difficulties.
- Knowledge of a range of relevant communication and dysphagia interventions relevant to the client group e.g. dementia, Parkinson's, COPD, stroke, etc.
- Knowledge of professional standards including record keeping.
- Knowledge of the role of Community Health Services.
- Knowledge of the importance of equality, diversity and inclusion and how this can be implemented in practice.
- Knowledge of national policies and procedures relevant to adults.
- An understanding of the principles of clinical governance.

- Knowledge of local services including voluntary agencies available.
