We are excited to offer a fixed term role (18 months) in care coordination service (CCS) as a Health and social care coordinator. The successful candidate will therefore naturally be expected to work in a highly multi-agency context to ensure vulnerable patients are able to navigate smoothly a complex health and social care system. While full details on the requirements of the role can be found in the job description and person specifications, experience in a health or social care setting, excellent organizational, interpersonal and communication skills are essential requirements for this role. A flexible approach to work in an ever changing health and social care system coupled with an interest in the needs of the most vulnerable people of our society is a highly desirable attribute.

The successful candidate will benefit from appropriate corporate and local induction, training, regular supervision and management support. The primary base of work for this role is currently in Featherstone Road Health Centre, Southall.

Main duties of the job

Working within a network of GP Practices to provide a central co-ordination role for patient identified throughout the health and social system and referred into the service

Responsible for a caseload of around 40/45 named patients with the aim of

o Enabling people to remain at home,

o Reducing unnecessary admissions to hospitals,

o Supporting early discharge from hospital,

o Improving the quality of care

o Encouraging independence.

Provide a single-point of access for service users and colleagues from the health and social care landscape involved in the care of the patient

Support the co-ordination of appropriate care packages for patients' identified needs ensuring to appropriately link services

Provide advocacy for service users where required making use of appropriate levels and channels of escalation

Facilitate smooth and planned discharge and handover between care settings across the health and social care system, including GP Practices, secondary care and community settings

Providing health and well-being support to patients

Delivering health and well-being training to health care professionals
