The Hull & East Riding Memory Assessment Service provides Dementia Diagnosis and treatment to those living in Hull & East Riding. You would be working with a dynamic, multi-disciplinary and multi agency team to deliver the diagnosis and treatment pathway to patients and their supporters.
There would be the expectation that the successful candidate would be an Independent Prescriber or willing to obtain this qualification.
The service is busy and provides assessment and treatment across various venues. Experience of working with those with Dementia is a must as is a passion for providing quality assessment and treatment.
Main duties of the job
The main duties of the role are: triage of referrals, assessment, formulation, delivery of diagnosis, provision of treatment interventions, forward care planning, responding to the changing needs of patients and their carers, forming good working relationships with partner services and the larger Older Peoples Mental Health Service. The safety of patients and their supporters is paramount as is the need for instilling hope in those diagnosed. Delivering education to promote the wellbeing of those living with Dementia and their supporters is also a part of the role.
This is also a supervisory role with responsibility for the clinical supervision of team members.

The applicant will be expected to have specialist nursing knowledge of working with people with Dementia. They must have knowledge of the relevant policies, guidelines and drivers.
The successful applicant will provide cognitive assessment, present at multi-disciplinary formulation, provide diagnostic feedback and identify appropriate interventions. You will be expected to prescribe Dementia medications and therefore must hold an Independent Prescriber qualification or be willing to study to achieve this and have the pre-requisites for this level of study. You must have the ability to consider the needs of those who care for the person living with Dementia and also the experience to be able to recognise the physical and mental health needs of those referred to the Service.
You will have the opportunity to develop leadership skills by providing clinical supervision and appraisal to team members and also ensuring local and national policies are implemented. You will be expected to represent the leadership of the Team in the absence of the Team Leader and Clinical Lead.
You must be confident lone working and also as part of the MDT. A willingness to work remotely and efficiently is a must. The service is busy and time management is vital to the implementation of the pathway.
