You may be required to work in other designated locations of the Trust as well as your primary base. The Northampton Specialist haemoglobinopathy team currently incorporates Northampton and Kettering, flexibility is required across the two main hospital sites.

Main duties of the job

To provide specialist nursing support ensuring the quality of care provision to adults who have Haemoglobinopathy disorders using unit protocols and trust policies and guidelines.

To work with the multi professional team to provide patient focused Haemoglobinopathy services, and to initiate different forms of treatment for adults and their families in a variety of settings.

To work with the multi professional team in Haemoglobinopathy services.

To provide specialist education, training and information to patients and families.

To work with the multi professional team to monitor patient condition.

To be responsible for nursing practice development.

The post holder will:

Maintain efficient and effective communication with the multidisciplinary team, both within NGH / KGH, the network and national service.

Develop and promote working partnerships between the Trust and academic staff at local Universities involved in the provision of nursing and multi professional programmes of education.

Be a visible, accessible, and assertive clinical expert to whom staff and users can turn to for assistance, expert advice, and support.

Job responsibilities

KEY RESULT AREAS

Take responsibility for ensuring effective communication between all service providers within the speciality.
  • Establish and maintain good positive working relationships with key working relations.
  • Facilitate training and support with programmes of education.
  • Actively participate in local, national, and international networking to maintain and develop expert level of knowledge in the speciality.
  • Ensure that the importance and implications of education, research, and ideas are raised in appropriate forums in the absence of manager.
  • Act up in the absence of the Haemaglobanopathy specialist nurse manager, assuming day to day line management of the team.
  • Work across NGH and KGH, and potentially other settings including community services, inpatient, and outpatient services

Service Delivery & Development
  • Manage a delegated caseload of patients, as appropriate within the speciality, delivering individualised and personalised direct patient care.
  • Provide clinical expertise, specialist advice and support across the whole spectrum of the service.
  • Demonstrate specialist knowledge, skills and innovative practice in the specialist area, ensuring the delivery of appropriate evidence-based care.
  • Support, assist and encourage the multi-disciplinary team in resolving complex patient problems, by the provision of new and innovative models of case management.
  • Promote the philosophy of patient centred care and establish and maintain a supportive relationship with the patient and their family at all times.

Governance
  • Support manager for ensuring appropriate systems are in place to evidence ongoing compliance with local and national policies as required for CQC standards.
  • Actively participate in audit and research and benchmarking activities to ensure effective safe and evidence based practice.
  • Promote and encourage nursing practice research in all relevant clinical areas.
  • Promote the speciality by publishing and presenting innovations and research findings locally and nationally.
  • Actively promote the specialist service throughout the Trust by establishing robust networks and links with Clinical Governance and Audit teams
  • Support manager with acting on audits and research findings to inform and change practice as appropriate.
  • Support manager as a clinical expert provide advice and support to assist with the management of complaints relating to the speciality and implement action plans or change of the service.

Patient/Customer Service
  • Promote the philosophy of patient centred care and establish and maintain a supportive relationship with the patient and their family at all times.
  • Promote patient and public involvement and enhancing their experience by supporting and supporting manager with implementing new initiatives relating to patient centred care.
  • Provide clinical expertise, specialist advice and support across the whole spectrum of the service within own scope.
  • Demonstrate specialist knowledge, skills and innovative practice in the specialist area, ensuring the delivery of appropriate evidence based care.

GENERAL
