An opportunity has arisen for a highly motivated and enthusiastic Band 6 Nurse Specialist to join the Community Nurses Heart Failure team.

We are seeking to recruit a motivated experienced Heart Failure Nurse with a genuine interest in pursuing a career in this area. Ideally, the applicant should have extensive experience working as a Band 5 registered nurse in the field of Cardiology/Cardiothoracic.

Applicants should be willing to undertake independent prescribing and clinical examination qualifications.

Main duties of the job

The successful post holder will be expected to demonstrate initiative, autonomy and independence in the development of this expanding service and be responsible for assessing and managing pathways for patients and their families with definite and possible Heart Failure conditions, involving travel and working between the participating sites.

Applicants will be involved in the collection and monitoring of National Heart Failure Audit tool to ensure Trust compliance with local CQUIN targets.

The role includes all aspects of heart failure nursing including assessment, providing clinical advice, delivering training, writing /reviewing policies and guidelines. Applicants must have excellent communication skills, be a team player and be able to work autonomously.

Excellent communication and organisational skills are essential as the successful applicant will act as a source of expertise and knowledge for patients, relatives and health professionals, and be able to identify patients and relatives individual specific needs. You will be a point of contact for family support for patients with heart failure, providing advice, screening information and supportive counselling as required. You will be responsible for the management of nurse led clinics and your own caseload, working autonomously making independent decisions, providing feedback to the lead Consultant Cardiologist regarding the decisions made.

You will have the ability to work effectively within a team as the role also involves providing support to the Cardiologists in the care of patients with Heart Failure conditions, and liaising with other specialties including Palliative care, Cardiac Physiologists, Echocardiography, Surgeons, District Nurses, Coroners and General Practitioners when required.

Person Specification

Education and Qualifications

Essential

  • RGN Level one
  • NMC registered
  • Assessing qualification and evidence of teaching ability
  • Continuous professional development
  • First degree as a minimum or working towards


Desirable

  • Non-Medical Prescribing (or willing to undertake)
  • Advanced Practice and Consultation Skills (or willing to undertake)


Experience

Essential

  • Extensive post registration experience
  • Extensive relevant experience at Band 5 within a cardiology/cardiothoracic setting
  • Ability to work, motivate and develop a professional team


Desirable

  • Experience as a band 6 community heart failure specialist nurse
  • Specialist training relevant to role: Counselling skills


Skills and Knowledge

Essential

  • ECG Interpretation or working towards.
  • Ability to communicate effectively including active listening, writing, and verbally, with individuals on a one-to-one basis and with groups.


Desirable

  • Cannulation / Venepuncture or working towards.
