An exciting opportunity has come about for an experienced nurse to work within the Specialist Community Palliative Care CNS team in a new role of Triage nurse.

We have a vacancy for either full or part time Band 6 nurses to work as part of the CNS team as a dedicated Palliative Care Triage nurse.

There is a high level of complexity and demand on the specialist CNS team resulting in a significant need for effective clinical triage at first point of contact. The post holder will be part of the CNS team but work in a supportive role alongside the CNS to clinically triage new patients and urgent patients into the service. They will have good clinical assessment skills, be able to make clinical decisions and hold strong skills in prioritising workloads. The post holder will form strong links with the wider MDT within palliative care and the community setting to ensure effective patient triage.

Main duties of the job

  • Act as the first point of clinical contact for the patient, following patient referral.
  • Identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professionals.
  • Undertake daily effective and comprehensive triage of all new referrals to the Community Palliative Care CNS team, including contacting the referrer for additional accurate information to ensure a holistic understanding of patient need.
  • Carry out holistic initial assessment via the telephone, safety netting, and providing any initial support and treatment.
  • Support with daily Palliative Care Virtual Wards and offer support to the CNS team in office-based patient admin, such as ordering equipment, providing packages of care and telephone support to ongoing patients.
  • Triage messages from patients on the current caseload to offer clinical support, advice and assessment, prioritising effectively.

Job responsibilities

  • To play a lead role in receiving and managing referrals made to the Community CNS team identifying appropriateness, urgency, and level of priority.
  • Triage all incoming referrals considering patients stated symptoms to determine the appropriate level of care required to safely meet the patients specialist palliative care needs.
  • Utilise agreed resources and guidelines to effectively assess, prioritise, advise, schedule patient care management or refer patient when necessary to other appropriate health and social care agencies.
  • To ensure a timely, responsive, supportive and effective service for the benefit of all stakeholders involved in the patient referral.
  • Work collaboratively with internal and external partners to support thecoordination and management of patients referred to the CNS team, and ensure care is planned to meet the individualised and dynamic needs of the patients, and those close to them.
  • To be able to conduct comprehensive holistic triage assessments over the telephone with patients, relatives and professionals to inform clinical decision making and prioritising the urgency of referrals.
  • Undertake specialist assessments of patients with moderate/complex palliative care needs, providing education for patients, relatives and carers.
  • Receive and manage referrals to the service and discharge patients appropriately, in line with agreed service criteria and utilising palliative care assessments.
  • Communicate effectively with patients, relatives and carers using specialist skills e.g. advanced communication skills as necessary.
