The post holder will carry out nursing duties in accordance with the NMC Code of Professional Conduct and ensure all Professional Standards and Trust Policies are complied with.
Main duties of the job
Use specialist knowledge, skills and experience to assess, plan, implement and evaluate programmes of care to meet identified needs with the aim of promoting positive health.
To have an awareness of clinical effectiveness and ensure interventions are based on research findings or evidence-based practice and linked to outcome measures.
Ensure all medication interventions/practices comply with the Trust's Medicines Management Policy and the NMC Standard for Administration of Medicines.
Be responsible for the ongoing assessment and treatment of patients ensuring emotional and professional support to patients and carers during assessment and treatment.
Ensure the privacy and dignity of patients and their carers/families is respected at all times.
In accordance with Trust policies and procedures and NMC record keeping guidelines, ensure that Nurses and Health Care Support Workers maintain accurate documentation and record keeping.
Be involved in discharge planning and liaise with carer/s and outside agencies.
Positively generate good relations and communications with patients' relatives/next of kin, ensuring that they are informed of the condition, progress and aftercare arrangements of their relative or friend receiving care.
Participate in liaison with Community Teams, Health Visitors and Social Services and General Practitioners in early preparation for continuity of care following discharge of patients.

Participate in the Appraisal process
Maintain a Personal Professional Portfolio
Access and provide Clinical Supervision in accordance with Trust Policy and NMC guidelines
Accept personal responsibility for registration in line with NMC policies and procedures.
Attend Professional Nurses meetings and ensure professional knowledge and competencies are maintained.
The post holder will contribute to the development of team members/others.
Teach and share knowledge with team members and other professionals
Assess and/or mentor learner nurses allocated to the service in liaison with the appropriate educational establishment.
Delegation of work to a Health Care Support worker and junior staff.
Ensure care plans are regularly evaluated through to discharge.
Participate with quality assurance initiatives which lead to enhanced service delivery to patients, carers and extended agencies / providers.
Accept responsibility for the day to day management of clinical area for delegated spans of duty ensuring that high standards of nursing practice are maintained.
Ensure safe custody of client records and the appreciation of the need for confidentiality at all times in accordance with Trust policies and procedures including national guidance, e.g Caldicott.
In accordance with Trust policies and procedures deal with all complaints appropriately in liaison with approved Trust departments, carers, advocates, patients and Customer Services
Promote and lead the use of evidence based practice as a vehicle for clinical intervention and risk management.
Promote good team working and effective communication that is central to the delivery of a high quality service
