Care Home in Reach team, 37.5 HOURS PER WEEK We have an exciting opportunity for a Qualified Nurse to join our well-established Care Home In-Reach Team which is a countywide service. The team accepts referrals for individuals of any age with a dementia diagnosis/ memory issues and functional mental health illness over the age of 65. The team provides training to care home staff. The service is open Monday to Friday 9-5 excluding bank holidays. Flexible working patterns will be considered. The team is based in the grounds of the Belmont Abbey which is peaceful rural location, 2 miles from the main city of Hereford. There is plenty of free parking. Main duties of the job
- Manage a caseload of patients to ensure they receive a timely assessment, diagnosis and treatment
- Monitor treatments
- Effective and timely discharge planning and coordination.
- Undertake and understand physical health observations and take appropriate action where necessary.
- Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.
- Apply a high level of understanding of the effect of dementia and provide advice on management of the condition, healthy lifestyles and well-being.
