The role of Community Mental Health Nurse is within the Care Management and Review Team. This is a multidisciplinary Health and Social Care Team, based within the Adult Community Mental Health teams in Oldham . The team provides services for adults with severe and enduring mental health conditions, through Health and Social Care Assessments and interventions and Care Plans, primarily through commissioned services.
Main duties of the job
The role involves undertaking Care Act assessments, reviewing after-care eligibility , commissioning care packages, and attending outpatients appointments as appropriate. The post holder may also be required to complete social circumstances reports for Managers Hearings or Tribunals. For out of borough placements and packages of care, an element of joint working will be required with the allocated Care Coordinator as part of the review process. Similarly, for in borough placements, an element of joint working with psychiatry, and/or Treatment clinic will be required as part of the review. Team members participate in a Duty Rota, and provide short term crisis response or relapse prevention interventions. The applicant may be needed to provide information on medication and treatments, to Social work members of the team. There are also opportunities to participate with input into the Treatment team to maintain any clinical skills.

Person Specification Registration Essential
- Registered Mental Health Nurse, with current registration with regulatory body( NMC)
- Substantial experience of post registration practice
- Experience of working within a community setting
